| 摘 要： | 目的:探讨hMSH2/hMLH1蛋白表达与慢性血吸虫病并结直肠癌之间的相关性及其可能的作用机制.方法:应用免疫组化SP法检测67例慢性血吸虫病并结直肠癌患者癌变组织及癌旁正常组织中hMSH2/hMLH1蛋白的表达情况.结果:慢性血吸虫病并结直肠癌患者癌变组织中hMSH2/hMLH1蛋白表达丢失率为28.4%,而癌旁正常结直肠组织中表达丢失率为1.5%(P=0.000);在黏液腺癌中hMSH2/hMLH1蛋白表达明显减低(P<0.05);逐步回归法进行Logistic多因素分析结果显示组织学分型是hMSH2/hMLH1表达缺失的唯一危险因素(OR=3.333,95%C 1.096～10.141,P=0.034).结论:hMSH2/hMLH1蛋白缺失与结直肠癌组织学分型有关;hMSH2/hMLH1蛋白缺失在慢性血吸虫病并结直肠癌中起重要作用,微卫星不稳定性可能参与了慢性血吸虫肠病发展为结直肠癌的恶性转化过程.
