慢性乙型肝炎患者外周血CD80、CD86及其受体CD28的表达

目的研究慢性乙型肝炎患者外周血CD80、CD86及其受体CD28与其病情状态、乙型肝炎病毒复制的关系。方法用流式细胞术检测了24名健康体检者和92例慢性乙型肝炎患者外周血CD80、CD86、CD28的表达和CD8^＋CD28^＋、CD8^＋CD28^-亚群淋巴细胞，并与病毒载量和乙型肝炎e抗原（HBeAg）进行相关分析。结果慢性乙型肝炎CD80和CD86表达明显高于正常对照组和乙型肝炎病毒携带组（P〈0．01）；肝炎后肝硬化组明显高于慢性乙型肝炎组（P〈0．05）；慢性乙型肝炎轻、中、重度3组间差异也有统计学意义。慢性乙型肝炎组CD28的表达明显低于正常对照组和乙型肝炎病毒携带组（P〈0．05）。病毒载量高低和HBeAg是否阳性与外周血CD80、CD86、CD28及CD8^＋CD28^＋和CD8^＋CD28^-亚群淋巴细胞差异无统计学意义。结论CD80、CD86及受体CD28可能在慢性乙型肝炎免疫损伤过程中起重要作用，但与血清中病毒载量的变化和HBeAg阳性与否无关。

Study of the expression of CD80, CD86 and its receptor CD28 in peripheral blood of patients with chronic hepatitis B

Objective To Study the relationship between CD80, CD86 and its receptor CD28 in peripheral blood of chronic hepatitis B patients with clinical condition and the replication of hepatitis B virus. Methods CD80, CD86, CD28 and CD8^＋/CD28^＋ , CD8^＋/CD28^- in peripheral blood cells were detected by flow cytometry in 92 patients with chronic hepatitis B and 24 healthy volunteers and the relationships between them and hepatitis virus quantity and HBeAg were analyzed. Results The percentage of CD80 and CD86 in patients with active chronic hepatitis B was significantly h;,gher than that of chronic hepatitis B virus carriers and the controls（ P 〈0.01 ） ,and that in patients with liver cirrhosis was significantly higher than that of the patients with active chronic hepatitis B （P 〈 0.05）. The expression of CD80 and CD86 in patients with hepatitis B were different from that in mild, moderate, severe patients with active chronic hepatitis B. The percentage of CD28 in patients with active chronic hepatitis B was significantly lower than that of the hepatitis B virus carriers and the controls. There were no difference of CD80, CD86, CD28 and CD8^＋/CD28^＋ , CD8^＋/CD28^- in peripheral blood cells between the group of high viral quantity and low viral quantity and the group of HBeAg positive and HBeAg negative. Conclusions CD80, CD86 and its receptor CD28 play an important role in the chronic hepatitis B immunity damages. But there were not relationship between the expression of CD80, CD86, CD28 and CD8^＋/CD28^＋, CD8^＋/CD28^- in peripheral blood cells and the change of hepatitis virus quantity and HBeAg in serum.
