卒中恢复期甲状腺激素水平与认知功能的相关性研究

摘    要:目的 探讨卒中恢复期患者甲状腺激素水平与认知功能的相关性,为认知功能康复提供参考。方法 前瞻性连续纳入2018年12月-2020年1月于石家庄市人民医院康复医学科收治的卒中恢复期患者为研究对象,根据MMSE评分将患者分为认知功能损害组和无认知功能损害组。比较两组甲状腺激素各项指标水平,包括三碘甲状腺原氨酸(triiodothyronine,T3)、甲状腺素、游离三碘甲状腺原氨酸(free triiodothyronine,FT3)、游离甲状腺素及促甲状腺激素水平,采用多元线性回归分析甲状腺激素各项指标水平与MMSE评分的相关性。结果 最终纳入210例患者,平均年龄59.97±7.12岁,男性138例(65.7%),其中认知功能损害组146例,无认知功能损害组64例。认知功能损害组T3、FT3水平低于无认知功能损害组,差异有统计学意义。多元线性回归分析结果显示,卒中恢复期患者血清T3、FT3水平与MMSE评分呈正相关(β=0.389,P <0.001;β=0.237,P =0.014)。结论 卒中恢复期患者血清T3、FT3水平越低,提示认知功能损害越重。临床工作中应积极关注卒中恢复期患者甲状腺激素水平,以尽早给予全面地康复干预。

Abstract:Objective To investigate the correlation between serum thyroid hormones level and cognitivefunction in convalescent stroke patients, and to provide reference for cognitive functionrehabilitation.Methods This prospective study consecutively enrolled the convalescent stroke patients atDepartment of Rehabilitation of Shijiazhuang People's Hospital from December 2018 to January2020. The patients were divided into cognitive impairment and no cognitive impairment groupsaccording to MMSE score. The level of serum thyroid hormones, including triiodothyronine(T3), thyroxine, free triiodothyronine (FT3), free thyroxine and thyroid stimulating hormone werecompared between the two groups. The correlation between serum thyroid hormones level andMMSE score was analyzed by multivariate linear regression.Results Finally, 210 patients were included, with a mean age of 59.97±7.12 years old and 138males (65.7%), including 146 patients with cognitive impairment and 64 patients without cognitiveimpairment. The levels of T3 and FT3 in cognitive impairment group were lower than those in no cognitive impairment group, with statistical difference. Linear regression analysis showed thatserum T3 and FT3 levels in convalescent stroke patients were positively correlated with MMSEscore (β =0.389, P <0.001; β =0.237, P =0.014).Conclusions The lower the serum T3 and FT3 levels in convalescent stroke patients, the moresevere the cognitive impairment.
