缓释氟尿嘧啶在腹腔镜直肠癌根治术中的安全性及疗效

目的探讨腹腔镜下植入缓释氟尿嘧啶治疗直肠癌的安全性及疗效。方法回顾性分析2006年3月-2009年1月我院普通外科172例行腹腔镜辅助下直肠癌根治术患者,观察组90例,术中在腹腔镜下将缓释氟尿嘧啶撒布在盆腔的瘤床周围及肝脏表面进行区域化疗;对照组82例,未行区域化疗。结果两组患者术前、术后及组间肝、肾功能和白细胞比较差异无统计学意义（P〉0.05）。观察组术后1年、2年局部复发率分别为1.11%（1/90）、7.78%（7/90）,对照组分别为8.54%（7/82）、17.07%（14/82）,1年局部复发率两组比较差异有统计学意义（P〈0.05）。观察组术后1年、2年肝转移率分别为5.55%（5/90）、17.78%（16/90）,对照组分别为15.85%（13/82）、26.83%（22/82）,1年肝转移率两组比较差异有统计学意义（P〈0.05）。结论缓释氟尿嘧啶在腹腔镜下直肠癌根治术中进行区域性化疗安全有效,是减少直肠癌局部复发及肝脏转移的有效方法。
