摘    要:目的 利用经会阴三维断层超声成像技术评价肛提肌的收缩功能及经会阴二维超声平面观察在三种不同状态下(在静息、肛提肌最大收缩及Valsalva动作)的盆腔脏器运动变化。方法 选择盆腔器官脱(POP)垂患者64例作为POP组及30例妇科良性疾病患者作为对照组,对所有患者在三种状态(静息、肛提肌最大收缩及Valsalva动作)下进行数据采集,并进行统计学分析。结果 静息状态下两组盆膈裂孔面积差异无统计学差异(P>0.05),Valsalva动作下两组盆膈裂孔面积差异有统计学意义(P<0.05)。POP组患者肛提肌收缩前后的厚度差异无统计学意义(P>0.05),对照组肛提肌收缩厚度差异有统计学意义(P<0.05)。结论 应用经会阴三维断层超声技术可用于评价肛提肌收缩功能;经会阴二维超声技术可指导患者进行盆底康复。

Abstract:Objective To assess contraction of the levator ani function by using translabial tomographic ultrasound imaging (TUI), and to observe the changes of pelvic organ under rest, anus-contracting and Valsalva with two-dimensional ultrasound. Methods Pelvic organ prolapse (POP) group included 64 patients with POP, while control group included 30 patients with other benign gynecological diseases. The data of all patients under rest, anus-contracting and Valsalva were collected and performed statistically analyzed. Result At rest, there was no difference of levator hiatus size (LHS) between the two groups (P>0.05), while LHSs were significantly different between the two groups at Valsalva (P<0.05). In POP group, there was no difference of levator ani thicknes (LAT) before and after levator ani contraction (P>0.05), while there was significant difference of LAT in control group (P<0.05). Conclusion TUI can be used in evaluation on contraction of the levator ani function. Two-dimensional ultrasound can guide patients for pelvic floor rehabilitation.
