复杂性胃十二指肠溃疡穿孔修补术84例分析

目的：比较2种复杂性胃十二指肠溃疡穿孔修补手术的临床效果。方法：选取本院1988年1月～2009年12月收治的复杂性胃十二指肠溃疡穿孔患者84例,随机分为A组和B组各42例,分别进行不同的穿孔修补术,比较两组的临床疗效及术后并发症情况。结果：两组手术均顺利完成,A组总计发生术后并发症16例,占38.1%;B组总计发生术后并发症2例,占4.7%。两组比较,差异有统计学意义（P〈0.01）。A组住院时间为13～44 d,平均35.2 d;B组住院时间为12～23 d,平均15.8 d,明显优于A组,进食时间B组比A组早。结论：对于复杂性穿孔常规修补法往往穿孔,修补不完全,组织容易切割,术后并发症多。网膜堵入加放射状缝合固定修补法,治疗复杂性胃十二指肠溃疡穿孔效果理想,且并发症少、术后愈合快。

Complexity of gastric ulcer perforation repair of 84 cases

Objective： To compare the complexity of the repair of gastric ulcer perforation clinical treatment.Methods： Select the hospital since January 1988 to December 2009 admitted to the complexity of the 84 patients with gastric ulcer perforation,were randomly divided into A group and B group of all 42 cases,respectively different perforation repair,compared the two group efficacy and postoperative complications.Results： Both operations were successfully completed,a group of total postoperative complications occurred in 16 cases,accounting for 38.1%;B group total postoperative complications occurred in 2 cases,accounting for 4.7%.Significant difference between the two groups（P0.01）.group A of hospital stay 13 to 44 days,an average was 35.2 days;group B of hospital stay 12 to 23 days,an average was 15.8 days,significantly better than the group A.Conclusion： Omental plugging into the patch plus radial suture fixation of complex treatment of gastric ulcer perforation satisfactory results,and fewer complications,faster healing after surgery,is worthy of clinical use.
