摘    要:目的:应用HRT-Ⅱ对慢性闭角型青光眼早期与晚期患者进行检查,比较两期各指标的差异,指导临床随访治疗等。方法:对于确诊的慢性闭角型青光眼进行HRT-Ⅱ检查,得到早期与晚期视盘参数,对其盘沿面积、盘沿体积、视杯形态测量、轮廓线高度变化、视神经纤维层厚度进行比较分析。结果:对于每位患者的早期与晚期两组HRT-Ⅱ视盘参数分析结果显示,其盘沿面积、盘沿体积、视杯形态测量、视神经纤维层厚度早期与晚期比较均有极显著差异(P<0.01),但轮廓线高度变化两组无差异,其(P>0.05)。对于慢性闭角型青光眼晚期其神经损害明显存在,其治疗目标应是尽量减少神经损害的发生,同时提示我们在慢性期神经损害并未消失。结论:维持正常眼压水平,定期进行视野及HRT-Ⅱ检查,追踪其神经改变,对早期发现慢性期的病理改变,从而挽救视力具有重要意义。

Abstract:AIM:To analyze the difference of optic disc parameters by HRT-II in the patients at early and late stages of chronic angle-closure glaucoma.METHODS:The optic disk parameters of the patients at early and late stages of chronic angle-closure glaucoma were measured on both eyes by Heidelberg Retina Tomograph-Ⅱ(HRT-Ⅱ).RESULTS:The optic disk parameters at early and late stages were compared for each patient.There were statistically significant differences in rim area(RA),rim volume(RV),cup shape measure(CSM) and retinal nerve fiber layer(RNFL) thickness determined by HRT-II between early stage parameters and late stage ones(P<0.01),but there was no significant difference in height variation contour(HVC) between early stage outcome and late stage one(P>0.05).CONCLUSION:Maintenance of normal intraocular pressure,periodical examinations of visual field and HRI-II,and tracking the changes of neuropathy have great significance for early detection of chronic pathologic changes and saving the visual acuity.
