ADAMTS1在结直肠癌组织中的表达及其与预后的关系

摘    要:目的探讨含血小板结合蛋白基序1的解聚蛋白样金属蛋白酶(a disintegrin and metalloproteinase with thrombospondin type I motif,ADAMTS1)在结直肠癌组织中的表达情况,并分析其与结直肠癌临床病理特征及预后的关系。方法采用免疫组化染色SP法检测65例结直肠癌组织和癌旁组织中ADAMTS1的表达,采用妒检验分析ADAMTS1表达与结直肠癌临床病理特征的关系,采用Cox比例风险模型分析ADAMTS1表达及其他临床病理特征与结直肠癌预后的关系。结果结直肠癌组织中ADAMTS1的表达阳性率为40%(26/65),癌旁组织为85%(55/65),结直肠癌组织较低(X=27.546,P〈0.001)。有淋巴结转移的结直肠癌组织其ADAMTS1的表达阳性率低于无淋巴结转移者(X=5.329,P=0.021)。ADAMTS1表达阴性结直肠癌患者的中位生存时间为27个月,而ADAMTS1表达阳性者为70个月,ADAMTS1表达阳性者的生存情况较好(X=10.151,P=0.001)。Cox比例风险模型分析结果显示,TNM分期(RR=3.782,95% CI:1.509-9.476,P=0.005)、淋巴结转移(RR=3.107,95% CI:1.186-8.138,P=0.021)和ADAMTS1表达情况(RR=2.020,95% CI:1.071-3.809,P=-0.030)均影响结直肠癌患者的预后,Ⅰ-Ⅱ期、无淋巴结转移及ADAMTSI表达阳性者的预后较好。结论ADAMTS1在结直肠癌组织中呈低表达;ADAMTS1表达与结直肠癌的淋巴结转移有关;ADAMTS1表达阳性结直肠癌患者的预后好于ADAMTS1表达阴性者,提示其是预测结直肠癌患者预后的重要指标。

Expression of ADAMTS1 in Colorectal Cancer Tissues and Its Correlation with Prognosis

Abstract:Objective To investigate the expression of a disintegrin and metalloproteinase with thrombospondin type I motif (ADAMTS1) in colorectal cancer tissues, and to study the relationship with clinicopathological features and prognosis of it. Methods Expression of ADAMTS1 was evaluated by immunohistochemistry (SP method) in 65 speci-mens, which obtained by resection from patients with colorectal cancer, including corresponding adjacent benign tissues. Chi-square test was used for analyzing the relationship between expression ofADAMTS 1 and clinicopathological features of colorectal cancer tissues. Cox proportional hazard model was used to explore the relationship between expression of ADAMTSI, othcr clinicopathological parameters, and paticnts' survival situation. Results The positive exprcssion ratc of ADAMTSI was 40% (26/65) in the colorectal cancer tissues and 85% (55/65) in the adjacent benign tissues, which was significantly higher in adjacent bcnign tissues (X2=27. 546, P〈0. 001). The positive expression rate of ADAMTSI was significantly lower in the colorcctal cancer tissucs with lymph node metastasis than that of the colorcctal cancer without lymph node mctastasis (X=5.329, P=0. 021). Results of survival analysis showcd that mcdian survival time wcrc 27 months in the ADAMTSl-negative group and 70 months in the ADAMTS l-positive group respectively, and the survival situation was bettcr in latter group (X=10. 15 i, P=0. 001). Results of multivariablc prognostic analysis of Cox propor-tional hazard model showed that colorectal cancer with I - I1 stage(RR=3.782, 95% CI: 1. 509-9. 476, P=0. 005), without lymph node metastasis (RR=3. 107, 95% CI: 1. 186-8. 138, P-0. 021), and with positive-expression of ADAMTS1 (RR=2. 020, 95% CI: 1. 071-3.809, P=0. 030) had better survival situation. Conelusions Expression of ADAMTS1 is down-regulated in colorectal cancer tissues and it is associated with lymph node metastasis. The prognosis of patients in ADAMTS 1-positive group is better than that of ADAMTS 1-negative group, suggesting that ADAMTS 1 may be an independent prognostic factor in colorectal cancer.
