高频超声引导下硬膜外分娩镇痛的研究

目的评估高频超声引导下硬膜外穿刺分娩镇痛的可行性。方法选择80例拟行硬膜外穿刺的患者,为分娩的初产妇,妊娠足月,年龄20~35岁。随机分为对照组和观察组,每组各40例。对照组采用常规方法进行定位穿刺,观察组采用超声引导穿刺硬膜外麻醉。穿刺成功后,观察两组穿刺麻醉后镇痛效果、麻醉药物使用量等指标,并进行比较。结果 80例硬膜外穿刺患者均穿刺成功。观察组较对照组减少麻醉药用量;观察组麻醉有效率为97.5%,对照组为92.5%;观察组优于对照组,差异有统计学意义(P0.05)。结论经高频超声引导下硬膜外麻醉穿刺成功率高,减少麻醉药用量,安全性好,是有一定的临床应用价值的方法。

Study on high-frequency ultrasound-guided epidural puncture in labor analgesia

Objective To evaluate the feasibility and prospect of high-frequency ultrasound- guided epidural puncture in labor analgesia. Methods All of 80 cases of pregnancy scheduled for epidural puncture were enrolled, which of primipara with full-term and aged 20- 35 years old. They were randomly divided into control group and observation group（40 cases/group）. The control group was performed conventional epidural puncture, whereas the observation group was performed ultrasound-guided epidural puncture in labor analgesia. After successful puncture, the anesthetic effectiveness, intraoperative anesthetic requirements and other indicators were observed and compared. Results All of 80 cases were punctured successfully. The observation group required less anesthetic dosage than control group, the anesthetic effectiveness rate in observation group and control group was 97.5 % and 92.5 %,respectively, and difference was significant between 2 groups（P〈0.05）. Conclusion It is demonstrated that the success rate of epidural puncture guide by high-frequency ultrasound is high, and could reduce the anesthetic dosage with security. It is a reliable method in clinical application value.
