2型糖尿病诊治状况分析

目的　了解医生对 2型糖尿病诊治标准调整的掌握程度 ,以提高医师对糖尿病诊治的水平 ,使患者能够得到早期规范治疗。方法　调阅本院 2 0 0 3年 7月至 9月出院的 2型糖尿病的病案 170例 ,记录患者的并发症及血糖、血压、血脂的检查、治疗情况 ,各数据经文字标化后输入电脑 ,由Excel 2 0 0 0软件统计分析。结果　临床医师对 2型糖尿病中并存胰岛素缺乏与胰岛素抵抗的认识不足 ,治疗仍以促胰岛素分泌的口服药物为主 (5 8 8% ) ,胰岛素制剂及改善胰岛素抵抗的药物使用率低。治疗中血糖监测不规范 ,半数以上患者血糖控制不良。仅 2 6 5 %的合并高血压者血压控制达标。 12 5例测定LDL C的患者中LDL C >3 35mmol/L的有 2 5 6 %。所有病例进行调脂者仅 14例 (8 2 % )。结论　加强临床医师对糖尿病进展的认识 ,确保糖尿病患者血糖、血脂、血压控制达标 ,对阻止糖尿病并发症的发生和发展有举足轻重的作用

Diagnosis and treatment of type 2 diabetes mellitus inpatients

Objective To assess physicians' mastering of changed diagnosis and treatment stardards for T_2DM(type 2 diabetes mellitus).Methods 170 records of T_2DM inpatients from July to September 2003 were examined in terms of blood glucose,blood pressure,serum lipoprotein as well as treatment.Results Most doctors do not know clearly about the fact that both insulin shortage and insulin resistance co-exist in T_2DM.So that oral hypoglycaemic agents were still used widely(as high as 58.8%),while insulin and drugs that moliorate insulin resistance were used not so often.During treatment,blood glucose check was not exercised normally.Over half patients' blood glucose under control.Only 26.5% patients with hypertension had checked with normal blood pressure.In the 125 inpatients who had tested low density lipoprotein(LDL-C),the 25.6% had LDL-C over 3.35mmol/L.Among all the patients only 14 had been treated with blood lipoprotein.Conclusion The doctors should pay more attention to the new standard about T_2DM and keep hypertension,blood glucose and blood lipoprotein of the patients under complete control.
