自拟半夏厚朴解郁汤联合团体心理治疗对胃癌术后伴抑郁障碍患者的疗效观察

目的探讨自拟半夏厚朴解郁汤联合团体心理治疗对胃癌术后伴抑郁障碍患者的疗效。方法选取2015年6月至2019年10月间山东省立第三医院收治的100例胃癌术后伴抑郁障碍患者,根据术后治疗方式不同分组,采用常规治疗和团队心理治疗的42例患者纳入对照组,采用常规治疗和团队心理治疗加用自拟半夏厚朴解郁汤治疗的58例患者纳入观察组。比较两组患者的汉密顿抑郁量表(HAMD)评分、应激反应及生活质量,并评估抑郁治疗效果。结果干预前,两组患者HAMD评分比较,差异无统计学意义(P>0.05);干预后,观察组患者各项HAMD评分因子和总分均低于对照组,差异均有统计学意义(P<0.05)。干预前,两组患者应激反应指标水平比较,差异无统计学意义(P>0.05);干预后,观察组患者各项应激指标水平均低于对照组,差异均有统计学意义(均P<0.05)。观察组患者各项生活质量评分均低于对照组,差异均有统计学意义(均P<0.05)。观察组患者抑郁治疗有效率为94.8%,高于对照组的81.0%,差异有统计学意义(P<0.05)。结论自拟半夏厚朴解郁汤联合团体心理治疗,可有效改善胃癌术后伴抑郁障碍患者的抑郁状况,减轻患者应激反应,提高临床疗效和生活质量。

Efficacy of self-formulated Banxia Houpu decoction for depression combined with group psychotherapy for post-operative depression in patients with gastric cancer

Objective To study the efficacy of self-formulated Banxia Houpi decoction for depression combined with group psychotherapy for depression in patients undergoing surgery for gastric cancer.Methods A total of 100 patients with depression after surgery for gastric cancer treated at Shandong Provincial Third Hospital from June 2015 to October 2019 were enrolled as the research subjects.They were divided into an observation group(58 patients)and a control group(42 patients)according to the postoperative treatment they received.The observation group received self-formulated Banxia Houpi decoction for depression combined with group psychotherapy and the control group conventional treatment plus group psychotherapy.Hamilton depression scale(HAMD)score,stress response and quality of life were compared between the two groups.The efficacy of different therapies was evaluated.Results Before the intervention,there was no significant difference in HAMD score or stress response level between the two groups(P>0.05).After the intervention,the overall score and scores of HAMD as well as stress response level were significantly lower in the observation group than in the control group(all P<0.05).The scores of quality of life were significantly lower in the observation group than in the control group(all P<0.05).The treatment efficacy rate was 94.8%in study group which was significantly higher than 81.0%of the control group(P<0.05).Conclusion Self-formulated Banxia Houpu decoction for depression combined with group psychotherapy can effectively improve depression,reduce alleviate stress response and increase quality of life in patients with gastric cancer complicated with depression after the surgery.
