纤支镜支气管灌洗对儿童难治性肺炎支原体肺炎临床症状、炎症因子水平的影响

目的研究纤支镜支气管灌洗对儿童难治性肺炎支原体肺炎临床症状、炎症因子水平的影响。方法随机数表法将本院2016年8月~2018年10月诊治的120例难治性肺炎支原体肺炎患儿分为观察组与对照组,各60例,对照组给予常规治疗,观察组给予纤支镜支气管肺泡灌洗。治疗7d天评价疗效和安全性,并比较两组患儿治疗前后的炎症因子水平变化。结果观察组治疗总有效率93.33%明显高于对照组的78.33%(P<0.05);观察组患儿退热时间、咳嗽消失时间以及住院时间明显短于对照组(P<0.05);与治疗前相比,两组患儿治疗后白细胞介素-6(IL-6)、白细胞介素-4(IL-4)、γ干扰素(IFN-γ)水平明显降低,且观察组治疗后IL-6、IL-4、IFN-γ水平明显低于对照组(P<0.05)。结论纤支镜支气管灌洗应用于儿童难治性肺炎支原体肺炎治疗可显著改善患儿临床症状,降低患儿炎症因子水平。

Effects of Bronchofiberoptic Lavage on Clinical Symptoms and Inflammatory Factors in Children with Refractory Mycoplasma Pneumoniae Pneumonia

Objective To study the effects of bronchofiberoptic lavage on clinical symptoms and inflammatory factors in children with refractory mycoplasma pneumoniae pneumonia.Methods According to the random number table method,120 children patients with refractory mycoplasma pneumoniae pneumonia diagnosed in the hospital from August 2016 to October 2018 were divided into observation group and control group,with 60 cases in each group.Control group was given routine treatment,and observation group was given bronchoalveolar lavage with fiberoptic bronchoscopy.The efficacy and safety were evaluated at 7d of treatment,and the changes of inflammatory factors before and after treatment were compared between the two groups.Results The total effective rate of treatment in observation group was significantly higher than that in control group(93.33%vs 78.33%)(P<0.05).The fever abatement time,cough disappearance time and hospital stay in observation group were significantly shorter than those in control group(P<0.05).Compared with before treatment,the levels of interleukin-6(IL-6),interleukin-4(IL-4)and interferon-γ(IFN-γ)in the two groups were significantly decreased after treatment,and the levels of IL-6,IL-4 and IFN-γin observation group were significantly lower than those in control group(P<0.05).Conclusion Bronchofiberoptic lavage in children with refractory mycoplasma pneumoniae pneumonia can significantly improve the clinical symptoms and reduce the levels of inflammatory factors of children patients.
