妊娠高血压并发HELLP综合征的临床特点

目的：探讨妊娠期高血压疾病并发溶血、肝酶升高、血小板减少（HELLP）综合征的临床特点及对母儿的影响.方法：对本院25例HELLP综合征患者（完全性HELIP综合征16例,部分性HELLP综合征9例）临床资料进行分析,比较两组的临床表现、实验室指标、母儿并发症和结局.结果：完全性HELLP组中,乳酸脱氢酶值等各种实验室指标显著高于部分性组 重度子痫前期或子痫、特殊表现及严重并发症发生率亦高于部分性组,差异均有显著性.两组剖宫产率、新生儿重度窒息率及围生儿病兄率比较,其差异无显著性.两组血小板和肝功能均于产后24 h开始恢复,产后1周左右恢复正常.结论：HELLP综合征严重危害母婴健康,完全性HELLP综合征较部分性综合征临床病情更加严重,应重视产前检查,及早发现HELLP综合征并早期诊断、及时治疗、适时终止妊娠,降低孕产妇及围产儿病死率.

Pregnancy-induced hypertension complicated with HELLP syndrome

Objective：Hypertensive disorders of pregnancy complicated by hemolysis, elevated liver enzymes, thrombocytopenia （HELLP） syndrome in the clinical characteristics and the impact on mother and child. Methods： 25 cases of HELLP syndrome （16 cases of complete HELLP syndrome,partial HELLP syndrome in 9 cases） clinical data were analyzed to compare the two groups of clinical manifestations, laboratory indicators, maternal complications and outcome of children. Results： Complete HELLP group, lactate dehydrogenase values, and other laboratory parameters were significantly higher than the partial group, severe preeclampsia or eclampsia, a special performance and serious complication rate was higher than the partial group, the differences signifi. Two cesarean section rate, neonatal asphyxia and perinatal child mortality rate comparisons, the difference was not statistically significant. Two groups of platelet and liver function were resumed 24 hours after birth, postnatal 1 week or so back to normal. Conclusion：Serious harm to maternal and child health HELLP syndrome, complete HELLP syndrome compared with partial syndrome more serious condition, should pay attention to prenatal care, Early detection of HELLP syndrome and early diagnosis, prompt treatment and timely termination of pregnancy, reducing maternal and perinatal mortality.
