柄状环乳晕切口在乳房脓肿切开引流术中的应用

摘    要:目的:探讨乳晕区脓肿的合适切口,以减轻切口疤痕。方法:55例乳房脓肿患者分为两组A组30例32侧乳房脓肿靠近乳晕区,在切开引流术中采用了柄状环乳晕切口;B组25例26侧乳房脓肿位于乳房周边,行放射状切口。通过术中排脓、适当冲洗、纱布充填引流和术后换药,10 d左右Ⅱ期缝合切口。结果:两组乳房脓肿术中排脓彻底,两组手术时间、出血量、乳头乳晕血运、清除脓腔间隔从切开引流到Ⅱ期缝合的时间间隔以及冲洗脓腔操作的难易程度方面无明显差异;柄状环乳晕切口疤痕不明显。结论:柄状环乳晕切口可应用于约20%的乳房脓肿患者,合理应用柄状环乳晕切口进行乳房脓肿的切开引流,不仅引流效果好,而且乳房切口疤痕不明显。

Abstract:Objective:To explore a suitable incision in breast abscess incision drainage and reduce scar.Methods:A total of 55 patients of breast abscess were randomly divided into two groups:30 cases with 32 breasts with the abscess near areola had breast abscess incision drainage by the haft-shape circum-areola incision;while 25 cases with 26 breasts with abscess surrounding breast had radial incision.Through purulent discharge,irrigating,gauze drainage and change of dressing,the wounds were sewed up atⅡ stage about 10 days later.Results:No significant difference was found in duration situation,surgery time,volume of bleeding,blood supply of nipple and areola,interval tom from incision to suture,and the procedure of abscess cavity clearance.The scar was abated by the haft-shape circum-areola incision.Conclusions:Haft-shape circum-areola incision could be applied in treatment for about 20% breast abscess patients.A sufficient drainage and abated scar can be achieved in breast abscess by reasonable application of the haft-shape circum-areola incision.
