脑膜瘤及相关疾病CT、MR和手术误诊分析

目的：研究脑膜瘤CT、MR和手术误诊的原因及其合理应用。材料与方法：收集脑膜瘤及相关误诊疾病共22例,分析CT、MR和手术误诊的主要原因,以及作出正确诊断的检查方法及其原因。结果：22例误诊中,作CT20例,误诊17例。作MR7例,误诊5例;并用CT和MR5例,其中2例CT错误,MR正确;3例MR错误,CT正确,CT误诊原因有“等密度或较小病变未作增强等７条。ＭＲ误诊原因有”不能确切识别肿瘤钙化“等４条。手术误诊主因受既往手术或细胞学结果的影响。结论：（１）Ｙ国防止脑膜瘤误诊,ＣＴ平扫必须补加增强扫描。（２）ＣＴ和ＭＲ联合应用可取长补短,避免误诊。（３）手术可纠正大部分影像学误诊。

An analysis of misdiagnosis by CT, MR and surgery in Meningioma and related diseasis

Purpose:To study the causes of misdiagnosis by CT, MR and surgical operation as well astheir rational applications.Materials and Methods:22 cases of misdiagnosed meningioma and relate diseaseswere collected and analysed:1The factors that give rise to misdiagnose by,CT MR, and surgery. 2The rationale and methods for accurate diagnosis.Results:Among the 22 cases , CT was performed in 20, 17 with error, andMR in 7, 5 with error. CT together with MR in 5, misdiagnosis was made by CT in 2, which were corrected justby MR;while erroreous diagnosis were made by MR in 3 that put right by CT. The causes of 7 misdiagnosis re-sulting from CT were considered as:"Didn't administrate contrast medium to isodensitive or small lesions" etc:and the 4 from MR, as "can't identified exactly the calcification in the tumor" etc; while from surgeon,mostlyaffected by the results of previous surgery and preoperative biopsy finding. Conclusion: To prevent from mis-diagnosis, enhancement must be made after CY plain scan.2With combined application of CT and MR, the er-roneous result can be avoided. 3 In most cases, imaging misdiagnosis can be redressed by surgery.
