218例超声引导下经皮肝穿刺活检术与慢性肝病临床诊断的相关性分析

目的 分析慢性肝病临床诊断结果与超声引导下经皮肝穿刺组织病理活检结果的一致性,为超声引导下经皮肝穿刺活检术对慢性肝病临床病诊断提供参考。方法 选择安徽中医药大学第一附属医院感染科2011年11月至2016年10月期间的218例慢性肝病患者,采用彩超引导下行肝脏病变处穿刺活检术,常规做苏木精—伊红(HE)染色、Masson及网状纤维染色,根据患者病情需要加做免疫组织化学及特殊染色,分析病理结果与临床诊断的相关性。结果 代偿期肝硬化(HBV)携带者、慢性乙型肝炎轻度、慢性丙型肝炎、脂肪性肝病、乙肝肝硬化(代偿期)与病理诊断分级的符合率分别为74.3%、78.7%、73.3%、80.6%和67.9%。结论 超声引导下经皮肝穿刺活检在慢性肝脏疾病诊断中具有重要的参考价值。

Correlation analysis of 218 cases of percutaneous liver biopsy under ultrasound guidance and clinical diagnosis of chronic liver disease

Objective To provide reference for the diagnosis of chronic liver disease by ultrasound guided percutaneous liver biopsy through the consistency analysis between clinical diagnosis of chronic liver disease and the results of ultrasound guided percutaneous liver biopsy.Methods Two hundred and eighteen cases of patients with chronic liver disease in The First Affiliated Hospital of Anhui University of Traditional Chinese Medicine from November 2011 to November 2016 were chosen as the objects of ultrasound guided biopsy of liver lesions biopsy.Conventional HE staining,Masson staining and reticular fiber staining were applied.Meanwhile,chemical and special staining immunohistochemistry were tested according to the condition of patients,and the correlation between pathological results and clinical diagnosis were analyzed.Results The compliance rates between clinical and pathological diagnosis of the HBV carriers,mild chronic hepatitis Bpatients,chronic hepatitis C,fatty liver disease and compensatory cirrhosis were 74.3%,78.7%,73.3%,80.6% and 67.9%,respectively.Conclusion Ultrasound guided percutaneous liver biopsy has important reference value in the clinical diagnosis of chronic liver disease.
