欣母沛用于预防前置胎盘剖宫产产后出血132例疗效分析

目的观察欣母沛用于前置胎盘剖宫产产后出血的疗效。方法将132例因前置胎盘行腹部子宫下段剖宫产手术的患者按随机数字表法分为观察组(72例)和对照组(60例)。观察组在剖宫产术中胎儿娩出后即在子宫体部注射缩宫素20U,静脉注射缩宫素20U,同时肌内注射欣母沛250μg。单次注射效果不佳时,可于15min后重复注射1次。总剂量不超过2mg。对照组在剖宫产术中胎儿娩出后即在子宫体部注射缩宫素20U,静脉滴注缩宫素20U。对2组患者术中,术后2、24h的出血量及产后出血情况进行比较。结果观察组产后出血8例,占11.11%,对照组产后出血22例,占36.67%,2组比较差异有统计学意义(均P<0.05);观察组术中,术后2、24h的出血量明显低于对照组(均P<0.05)。结论欣母沛是预防前置胎盘剖宫产产后出血的一种有效方法。

Carboprost Tromethamine for Preventing Postpartum Hemorrhage Due to Placenta Previa at Cesarean Section in 132 Women

Objective To observe the curative effect of Hemabate（carboprost tromethamine） on postpartum hemorrhage due to placenta previa at cesarean section.Methods A total of 132 women undergoing lower uterine segment cesarean section due to placenta previa were randomly divided into observation group（n=72） and control group（n=60）.The observation group was given injection of oxytocin 20 U in uterine body,intravenous injection of oxytocin 20 U and intravenous injection of Hemabate 250 μg after babies were delivered by cesarean section.Injections were repeated after 15 minutes if single injection was ineffective（total doses lower than or equal to 2 mg）.The control group was given injection of oxytocin 20 U in uterine body and intravenous infusion of oxytocin 20 U after babies were delivered.The blood loss was compared between the two groups at 2 and 24 hours after operation.Results The incidence of postpartum hemorrhage in observation group（8 women,11.11%） was significantly lower than that in control group（22 women,36.67%）（P0.05）.In addition,observation group showed significantly less blood loss than control group at 2 and 24 hours after operation（all P0.05）.Conclusion Hemabate is effective in preventing postpartum hemorrhage due to placenta previa at cesarean section.
