唇部缺损整复方法探讨

目的探讨唇部缺损不同整复方法的适应证、疗效及预后。方法手术治疗38例患者,唇缺损占唇长1/3以内,行直接拉拢缝合;唇缺损占唇长1/3～1/2者,采用唇交叉组织瓣(Abbe-Estlander)法整复;唇缺损占唇长>1/2～2/3者,采用单侧或双侧唇颊滑行组织瓣整复;唇缺损超过唇长2/3或全唇缺损,采用扇形瓣颊侧转移法(Gillies)或鼻唇沟复合组织瓣整复。结果 38例各组织瓣均成活良好,术后患者面部功能及外形基本恢复。结论唇部缺损的整复,术式的选择应根据缺损的部位、大小、程度、形状、各瓣的特点,采用不同类型的整复方法,可获得较为满意的效果。

Restoration of Lip Defect

Objective To evaluate the indication,efficacy and prognosis of lip defect treated with different reconstructive methods.Methods Thirty-eight patients with lip defect underwent lip reconstruction.The defects less than 1/3 of lip length,from 1/3 to 1/2 of lip length,from 1/2 to 2/3 of lip length,and more than 2/3 of lip length were treated with direct suture,cross-lip flaps（Estlander Abbe flaps）,bilateral or unilateral labial-buccal gliding flaps,and buccal fan-shaped flap transfer（Gillies） or Nasolabial fold composite tissue flaps,respectively.Results All flaps survived well and facial function and appearance recovered basically in all patients.Conclusion Lip reconstructive procedures for lip defect should be selected according to defect position,size,degree,shape,and flap characteristics to obtain a satisfactory treatment effect.
