脑小血管病患者中脑微出血与认知功能障碍的关系

摘    要:目的 探讨脑小血管病(SCVD)患者中脑微出血(CMBs)与认知障碍的相关性及影响认知障碍的危险因素。方法 选取2021年3月—2022年3月在本院收治的120例SCVD患者行前瞻性研究,其中33例纳入CMBs阳性组,87例纳入CMBs阴性组;入组后均进行认知功能评估,并分析脑小血管病患者CMBs及其他传统脑血管病危险因素与认知功能障碍的关系。结果 CMBs阳性组MMSE总分及其四个子项内容包括:定向力、注意力和计算力、回忆能力及语言能力评分均低于CMBs阴性组,差异有统计学意义(P<0.05);Logistic回归分析表明,CMBs阳性、Fazekas评分、年龄是认知功能损害的独立危险因素(P<0.05)。结论 SCVD患者微出血病变与认知障碍的发生密切相关。

Abstract:Objective To explore the correlation between cerebral microbleeds (CMBs) and cognitive impairment in patients with cerebral small-vessel disease (CSVD) and the risk factors affecting cognitive impairment, so as to provide a potential predictor for prevention of dementia. Methods A total of 120 SCVD patients admitted to our hospital from March 2021 to March 2022 were selected, including 33 patients in the CMBs positive group and 87 patients in the CMBs negative group. Cognitive function was assessed after the inclusion, and the relationship between CMBs and other traditional cerebrovascular disease risk factors and cognitive dysfunction was analyzed. Results As a result, the total MMSE score of the CMBs positive group and its four contents namely: orientation, attention, computation, recall, and language ability score significantly decreased as compared to those of the CMBs negative group, and the difference between the two groups was statistically significant (P=0.05). Logistic regression analysis showed that CMBs positive, Fazekas score and age were the main risk factors for cognitive impairment. Conclusion Microbleeding lesions in SCVD patients are closely related to the occurrence of cognitive impairment
