腹部多排螺旋CT血管成像在经动脉导管介入治疗肝癌中的应用价值

目的探讨多排螺旋CT血管成像(MSCTA)在肝癌介入治疗中的指导价值。方法对98例肝癌患者进行常规的多期增强扫描诊断后,在后处理工作站进行最大密度投影(MIP)、容积再现(VR)血管重建。分析腹腔干动脉开口位置和方向;肝癌肿块供血血管来源,设定介入治疗计划。结果MSCTA可以清晰显示全部病例的腹腔干动脉开口位置和方向;显示大部分(78例)肿瘤供血血管。根据多排螺旋CT(MSCT)提供的腹部血管及肿瘤供血血管信息,指导数字减影血管造影(DSA)机下进行肝癌经导管动脉化疗栓塞,所有病例均能准确栓塞肿瘤供血动脉。结论MSCTA能够容易地确定介入治疗相关血管开口位置,很好显示肝癌供血血管,以此为向导,有助于提高介入治疗的准确性和效率,从而提高了手术的成功率,肝癌肝动脉化疗栓塞(TACE)术前行MSCTA对指导TACE治疗大有益处。

Objective To evaluate the guidance value with MSCTA(Multislice helical computer-tomography angiography MSCTA) in the interventional treatment of liver cancer.Methods With 98 cases of liver cancer patient being made many-phase of enhancement scanning on the diagnosis,maximum density projection(MIP) and volume rendering(VR) for the blood vessel are reconstructed by the post-processing workstation.To analyze the coeliac trunk artery aperture position and the direction;and the origin of supply blood vessel of the tumor.To design involvement treatment plan.Results MSCTA can display clearly the direction and position of the coeliac trunk artery aperture in all the 98 cases(98 examples) the coeliac trunk artery aperture position and the direction;it can also show the supply blood vessel of the tumor in most cases(78 cases).Aaccording to the information of abdomen blood vessel and the supply blood vessel given by MSCTA Supplies the abdomen blood vessel and the supply blood vessel information according to MSCT,the involvement treatment can be guidinged the involvement treatment,the coincidence rate is 100%.Conclusion MSCTA can determine very easily the involvement treatment related blood vessel aperture position,the very good demonstration of liver cancer supply blood vessel,As a guide,It it is helpful in the enhancementenhancing the accuracy and the efficiency of involvement treatment accuracy and the efficiency,and thus furthermore it can raised success rate of the surgery.
