两种不同呼吸机在治疗慢性阻塞性肺疾病伴呼吸衰竭时的比较

目的通过对面罩机械通气治疗慢性阻塞性肺疾病(COPD)伴呼吸衰竭的患者进行回顾,比较两种不同呼吸机在应用中的区别,评价其临床意义。方法回顾我院近两年采用面罩机械通气治疗30例COPD伴呼吸衰竭患者取得成功者,根据所采用呼吸机分为BiPAP组16例和西门子组14例,6小时内定期记录病情变化,血气参数及治疗时间并加以比较。结果两组治疗后2小时内呼吸频率、心率下降,PaO2上升,6小时内患者症状、血气指标较治疗前全面改善;通气1小时BiPAP组PaO2高于西门子组(0.010.05),BiPAP组PaCO2高于西门子组(P<0.01)。结论两种呼吸机成功治疗COPD呼衰时所需时间接近,所存在的部分指标上的差异取决于呼吸机性能上的差异,相对于COPD患者所需的机械通气目标,其临床意义不大。

Effects of noninvasive mask ventilation in respiratory failure due to chronic obstructive pulmonary disease: a Comparison between two types of mechanical ventilator

Objective To compare the early effects of noninvasive mask ventilation in respiratory failure due to chronic obstructive pulmonary disease(COPD) between two types of mechanical ventilator.Methods 30 cases of COPD with respiratory failure gained benefits from noninvasive mask ventilation were retrospectivly divided into two groups: 16 cases of the BiPAP group and 14 cases of the Simens group.Symptoms and the blood gases analysis(BGA) were recorded and compared regularly in the first 6 hours of treatment.Results The respiratory rate (RR) and heart rate(HR) decreased and PaO_2 increased after 2 hours' treatment,symptoms and BGA improved greatly after 6 hours' treatment; PaO_2 in BiPAP group is higher than in Simens group after 1 hour's treatment(0.010.05),but PaCO_2 is higher in BiPAP group than in Simens groups (P<0.01).Conclusions Time to gained benefits from noninvasive mask ventilation is very close in two groups,mechanical characteristics of ventilators were contributed to differences between two groups.
