食管自发性破裂的诊断与治疗

目的:总结食管自发性破裂的原因、诊断及治疗方法,以提高诊治水平。方法:回顾性分析7例食管自发性破裂患者的临床资料,7例患者确诊后早期开胸修补食管破裂口及纵隔和胸腔引流。结果:6例患者误诊,所有患者开胸修补食管破裂口,均获得成功。结论:食管自发性破裂原因复杂,容易误诊,胸部CT对食管自发性破裂的诊断具有积极意义,一旦确诊早期开胸修补食管破裂口、充分的引流是治疗成功的关键。

Diagnosis and Treatment of Spontaneous Esophageal Rupture

Objective:To summarize the reasons,diagnosis and treatment of spontaneous esophageal rupture.Method:Clinical datas of 7 patients with spontaneous esophageal rupture were analysed from July 1994 to December 2006.7 patients underwent primary surgical repair and mediastinal and pleural drainage procedures immediately after diagnosis.Result:6 patients was misdiag-msed.All patients recovered very well after the operation.Conclusion:The reasons of spontaneous esophageal rupture are complicated.CT is emerging as a very useful tool to spontaneous esophageal rupture.The key point of the therapy is emergent primary surgical repair and sufficient mediastinal and pleural drainage immediately after diagnosis established.
