摘    要:目的探讨三维适形放疗(3D-CRT)对非小细胞肺癌(NSCLC)患者肺功能的影响。方法选取2011年6月-2013年12月在遂宁市中心医院治疗的80例NSCLC患者,采用3D-CRT进行治疗。通过分析急性放射性肺炎级别、肺功能、剂量体积直方图(DVH)参数的指标数据,探讨3D-CRT对NSCLC患者肺功能的影响。结果 80例患者均顺利完成放疗,其中有29例患者出现不同程度的放射性肺炎。放疗前后,用力肺活量(FVC)、1秒内用力呼气量(FEV1)及肺一氧化碳弥散量(DLCO)值均出现不同程度的变化。结论采用3D-CRT治疗NSCLC,会对患者的肺功能产生一定影响。因此要注意对患者放疗前后肺功能变化情况、DVH参数和放射性肺炎分级情况进行综合分析,以减少放疗对NSCLC患者肺功能的不良影响。

Abstract:Objective To discuss the impact of 3D-CRT on the pulmonary function of patients of NSCLC.Method To select 80 NSCLC patients having received treatment at the Suining Central Hospital from June2011 to December 2013,adopt 3D-CRT on them,and discuss the impact of 3D-CRT on the pulmonary function of NSCLC patients through analysis of the data of indexes of the grades of acute radiation induced pneumonia,pulmonary function,and DVH parameters.Results All 80 patients completed their treatment smoothly,and29 of them showed different degrees of radiation induced pneumonia.Changes of varying degrees occurred before and after the radiotherapy in the numbers of FVC,FEV1 and DLCO.Conclusion Treating NSCLC with3D-CRT produces certain impact on the pulmonary function of the patients.It is thus advised to monitor and comprehensively analyze the changes in patients'pulmonary function before and after the radiotherapy,DVH parameters,and the grades of radiation induced pneumonia so as to reduce negative impacts of the radiotherapy on the pulmonary function of NSCLC patients.
