左房M型超声评价冠心病患者左室舒张功能

目的：探讨左房Ｍ型超声指标评价冠心病患者左房舒张功能的价值。方法：获取左房Ｍ型超声图像,在左房排空早、晚期测算主动脉后壁运动斜率Ｅｓｌｏｐｅ、Ｌｓｌｏｐｅ及左房缩短分数ＦＳｇ、ＦＳａ;采用频谱多普靳超声测取二类瓣口舒张血流速度Ｅ、Ａ及Ｅ／Ａ比值,测取左室等容舒张时间（ＩＶＲＴ）;采用智能声学定量技术测取左室舒张早、晚期及全舒张期充盈量（ＥＤ－ＦＶ、ＬＤＦＶ、ＤＦＶ）。结果：冠心病患者按Ｅ／Ａ比值分

Evaluation of left Ventricular Diastolic Function in Coronary Artery Disease by left Atrial M-mode Echocardiography

To assess the value of left atrial M - mode echocardiography in evaluation left ventricular diastolic function Methods: We determined the left atrial (LA) M - mode echograms and Doppler transmitral flow velocity profilers in 40 patients with CAD and 30 normals. We calculated LA global and active fractional shortening and the slope of both early and late displacement of the posterior aortic wall during LA emptying by M -mode echocardiography. We calculated peak velocity of early (E) and atrial (A) filling, E to A ratio, and time of isovolu- mic relaxation (IVRT) by pulsed Doppler echocardiography.The filling volume of early and late and total diastole were measured by Acoustic Quan- tification (AQi).Result: Patients with CAD were divided into three groups according to their E/A ratio Compared with normal subjects, 1) abnor- mal relaxation as E/A ratio: IVRT were prolonged. FSg, Eslope, EDFV were decreased obviously, but FSa, LDFV were increased obviously. There was no obvious change in DFV, 2) "pseudonormalized " pattern as 1 < E/A ratio < 2: FSa, FSg, Eslope, Lslope, EDFV, LDFV and DFV both were reduced obvionsly. There was obvious prolongation in IVRT; 3) restrictive filling as E/A ratio 2. All indexes of patients in this group were decreased obviously. Conclusion: LA M - mode recordings provide a more reliable and direct noninvasive assessment of diastolic function in CAD than mitral Doppler indexes.
