穿孔素和颗粒溶素在多发性肌炎和皮肌炎中的表达

目的:观察穿孔素和颗粒溶素在多发性肌炎(PM)和皮肌炎(DM)患者血清中的表达水平及与疾病活动的关系。方法:通过ELISA方法测定炎性肌病(PM/DM)组、类风湿关节炎组和正常对照组血清中穿孔素和颗粒溶素表达的差异;比较PM和DM组中穿孔素和颗粒溶素表达是否有差异;分析穿孔素和颗粒溶素的表达与炎性肌病疗效的关系。结果:穿孔素和颗粒溶素在PM和DM患者间差异无统计学意义(P>0.05);穿孔素在炎性肌病组、对照组和类风湿关节炎组间表达无差异(P>0.05);颗粒溶素在炎性肌病组和类风湿关节炎组表达均较对照组高(P<0.01和P<0.05),而炎性肌病组和类风湿关节炎组间无差异(P>0.05);在炎性肌病组中颗粒溶素和穿孔素表达无相关性,并两者在激素敏感患者和抵抗患者间无差异。结论:穿孔素和颗粒溶素单独血清学的测定不能作为判断炎性肌病活动和预后的指标,有待从多方面进一步研究。

Perforin and granulysin expression in serum of patients with polymyositis or dermatomyositis

Objective:To determine the relationship of perforin and granulysin expression in the serum of patients with polymyositis(PM) or dermatomyositis(DM) and progression of the disease.Methods:Enzyme-linked immunosorbent assay(ELISA) was used to detect the expression of pore-forming protein and granulysin in patients with PM/DM,rheumatoid arthritis(RA) and healthy control subjects.The protein level was compared between the groups of PM and DM for the differences and examined for the level and progression of the disease.Results:No significant statistical difference was found regarding both perforin and granulysin levels in patients with PM/DM or single perforin expression in those with RA and controls(P>0.05),whereas granulysin was over-expressed in patients with RA or inflammatory myopathy as compared with control subjects(P<0.01 or P<0.05),and the protein remained similar within the RA and PM/DM groups(P>0.05).There was no correlation between perforin and granulysin in patients with PM/DM as well as those with response or resistance to corticosteroids.Conclusion:Our findings supported very least possibly simple levels of perforin and granulysin as a serologic marker to estimate the progression and prognosis of inflammatory myopathy,and the influential factors of the two proteins remain to be further determined in wider aspects.
