泪小管断裂置入硬膜外麻醉管和硅胶软管的比较

目的比较外伤性泪小管断裂置入硬膜外麻醉管和硅胶软管后再行吻合手术的治疗效果。方法选择本院收治的外伤性下泪小管断裂共54例(54眼)，在手术显微镜下寻找到泪小管断端后，30例置入硬膜外麻醉管，24例置入硅胶软管。然后断端用8—0可吸收缝线作非穿通性的间断缝合3～4针使之吻合，3月后拔管。随访观察泪道通畅情况。结果54例都能在手术显微镜下成功找到泪小管断端。硬膜外麻醉导管组中痊愈18例(60．00％)，好转10例(33．33％)，无效2例(6．67％)；硅胶软管组中痊愈16例(66．67％)，好转6例(25．00％)，无效2例(8．33％)。结论置入硬膜外麻醉管或硅胶软管后再行泪小管吻合手术可以有效治疗外伤性泪小管断裂，但两者各有利弊。

Comparison of implantation of epidural tube or silicage tube in cases of lacrimal canaliculi breakage

Objective To compare the clinical effects of implantation of epidural tube or silicage tube in cases of traumatic lacrimal canaliculi breakage. Methods 54 eyes (54 cases) of traumatic lacrimal canaliculi breakage were treated. 30 cases were treated by implanting of epidural tube, 24 cases were treated by implanting of silicage tube. The tube would be reserved for 3 months. Results All canaliculi breakage can be seen under the microscope. In the group of using epidural tube, 18 cases (60.00%) had been cured, 10 cases (33.33%) were utility and 2 cases (6.67%)had no effect; In the group of using silicage tube, 16 cases (66.67%) had been cured, 6 cases (25.00%) were utility and 2 cases (8.33%)had no effect. Conclusion Traumatic lacrimal canaliculi breakage can be treated by implanting of epidural tube or silicage tube, both of them can have good clinical effect and both have its advantages and disadvantages.
