非浓缩尿蛋白电泳在2型糖尿病肾病中的诊断价值

目的探讨非浓缩尿蛋白(UTP)电泳在2型糖尿病肾病中的诊断价值.方法应用十二烷基硫酸钠-琼脂糖凝胶电泳(SDS-AGE)技术,在法国Sebia电泳仪上检测分析70例2型糖尿病人尿微量蛋白的组成.结果 70例2型糖尿病人尿液经SDS-AGE 电泳发现:36例正常蛋白尿(UTP<120mg/L)组中,有2例肾小管型蛋白尿,1例肾小球性蛋白尿,33例生理性蛋白尿;15例微量蛋白尿(120300mg/L)组中,有1例肾小管性蛋白尿,9例肾小球性蛋白尿,7例混合性蛋白尿,2例生理性蛋白尿;与正常对照组比较,肾小管型蛋白尿、肾小球性蛋白尿和混合性蛋白尿患者的收缩压、总胆固醇(TCho)和载脂蛋白A1/B100有极显著差异(P<0.01),与舒张压、甘油三酯(TG)有显著性差异(P<0.05),与病程、年龄无显著性差异(P>0.05).结论尿蛋白电泳比传统总蛋白检测法估计肾脏损伤程度更为可靠;测定血压、Glu、TG、TCho和载脂蛋白A1/B100,联合尿蛋白分析对糖尿病早期肾损伤的诊断和监测有重要临床价值.

The diagnostic value of urine protein non-concentrated in the type 2 diabetes nephropathy

Objective To explore the diagnostic value of urine protein non concentrated in the type 2 diabetic nephropathy.Methods The activation of urine microprotein in 70 samples from the patients with type 2 diabetic nephropathy were analysed by the SDS AGE technique and the Freach sebia electrophoresis apparatus.Results In the 36 samples from the type 2 diabetes mellitus group with UTP<120mg/L,there are 2cases of tubular proteinuria,1 case of mixed type and 33 cases of physiological type;In the 15 samples from the 120300mg/L group,there are 2 cases of physiological,one of tubular,9 of glomerular and 7 of mixed type.As compared with the 20 cotrol subjects,the SBP,TCHO and ApoA1/B100 are significantly differences in the patients with tubular,glomerular and mixes proteinuria( P <0.01),MBP and TG are marked differences( P <0.05),but their disease duration and age are of no significant differences ( P >0.05).Conclusion Compared with urine,the urine protein eletiophoresis is more reiable.The monitoring of SBP,Glu,UTP,TCHO,A1/B100,combined with the examination of urine protein,has great significance for both the diagnosis and the monitoring of the early injury from the diabetes mellitus.
