鼻内镜鼻窦手术517例临床观察

目的 观察内镜手术治疗鼻窦炎、鼻息肉的疗效。方法 对517例(962侧)慢性鼻窦炎、鼻息肉患者行内镜鼻窦手术，术后随访半年以上。结果 治愈343例(66．3％)，好转125例(24．2％)，无效49例(9．5％)，总有效率90．5％。术中、术后并发症主要为眶纸样板损伤及鼻腔粘连。结论 鼻内镜手术治疗慢性鼻窦炎鼻息肉有良好的疗效，手术者的良好解剖学基础、术中妥善止血、鼻道窦口复合体的正确处理及术后定期随访是提高疗效、减少并发症的保证。

Clinical observation of endoscopic sinus surgery with 517 cases

Objective To observe the long term curative effect of endoscopic sinus surgery(ESS). Methods 517 cases(962 sides)of chronic sinusitis and nasal polyps were treated with ESS, and following up surveyed six months post operation. Results It was found that 343 cases were cured(66 3%),125 cases achieved symptomatic relief (24 2%),49 cases remained failure (9 5%), and the effective rate was 90 5%. Main oprative complications were adhesion of nasal cavity and damage of lamina orbitalis. Conclusion ESS is effective in the treatment of chronic sinusitis and nasal polyps. The success of opration is directly related to the excellent anatomic basis, and dealing with hemorrhage properly. It is important to treat ostiomeatal complex precisionly, and regular follow up could reduce operation complications.
