摘    要:目的 探讨细针吸取活检( F N A B) 细胞学检查对甲状腺乳头状癌的诊断价值。方法 以术后病理诊断为对照,光镜下观察不同级别乳头状癌的针吸涂片中的单个细胞参数、细胞团参数和继发性改变。结果 在126 例乳头状癌中,细胞学诊断正确者116 例( 占92 .1 % ) ,可疑者2 例( 占1 .6 % ) ,假阴性者8 例( 占6 .3 % ) 。在诊断正确的116 例中,细胞学分级正确者113 例( 占97 .4 % ) 。同期细胞学诊断为乳头状癌者未见假阳性。结论  F N A B 对甲状腺乳头状癌的定性和分级诊断均准确可靠。还探讨了假阴性的原因和诊断中应注意的问题。

Abstract:Objective To discuss the value of fine needle aspiration biopsy (FNAB) in cytological examination for thyroid papillary carcinoma. Methods Under a light microscope and in contrast with postoperative pathological diagnosis, we studied the changes of single cell, of cellular group and the secondary changes on FNAB smears of thyroid papillary carcinoma of different grades. Results In the 126 cases of thyroid papillary carcinoma, the cytological diagnosis was correct in 116 cases (92.1%), a cancer was suspected in 2 cases (1.6%) and 8 cases (6.3%) were false negative. Of the 116 cases correctly diagnosed, the cytological typing was correct in 113 cases (97.4%). No patients in those who were cytological diagnosed as papillary carcinoma were false positive. Conclusions FNAB cytological examination is reliable for diagnosis and grade of thyroid papillary carcinoma. The false negative causes and the attention points in the diagnostic procedure are discussed.
