老年卵巢上皮癌患者的治疗及预后分析

目的 :研究老年卵巢上皮癌患者的治疗及预后。方法 :卵巢上皮癌 84例接受瘤体减灭术及CAP化疗 环磷酰胺 (CTX)、阿霉素 (ADR)、卡铂 (CP) ]。按年龄将患者分为 <4 0岁、4 1～ 50岁、51～ 60岁、≥ 61岁 4组 ,再按临床分期分为Ⅰ～Ⅳ期 4小组 ,比较不同年龄组 ,不同临床分期患者的 3年复发率及生存率。结果 :4年龄组 3年生存率依次为 :Ⅰ～Ⅱ期 75.0 %、10 0 .0 %、87.5%、66.7% (P >0 .5) ;Ⅲ～Ⅳ期 50 .0 %、54.9%、71.4 %、64.0 % (P >0 .5)。 3年复发率Ⅰ～Ⅱ期为 2 5.0 %、0、4 1.7%、33.3% (P >0 .5) ;Ⅲ～Ⅳ期为 66.7%、57.4 %、73.2 %、32 .0 % (P >0 .5)。结论 :年龄对卵巢上皮癌患者的复发率及生存率无显著影响。老年卵巢上皮癌患者在一般情况下 ,仍应积极手术及化疗。老年患者可承担手术及化疗的毒性作用 ,其生存率及复发率与年轻患者差异无显著性。支持治疗对一般情况较差的患者很重要。老年卵巢上皮癌患者与年轻患者比可能复发率高 ,预后差

Study on the treatment and pr og nosis for aged with ovarian epithelial cancer.

Objective:To study the t reatment and prognosis for aged with ovarian epithelial cancer.Methods: 84 patients with stage Ⅰ～Ⅳ ovarian epithelial carcinoma partic ipated in this trial.They were divided into four groups according to the age:＜ 40y;41～50y;51～60y;≥61y.Each group was subdivided according to their clinical stage.All patients received cytoreductive surgery and standard carboplatin based chemotherapy. Results:The 3a survival rate of the four groups(＜40y,41y～50y,51y～60y,≥61y)were:Stage Ⅰ～Ⅱ75.0％,100％,87.5％,67.0％(P＞0.5);Stage Ⅲ～Ⅳ 50.0％，54.9％，71.4％，64.0％(P＞0.5).The 3a recurrence rates of the four groups were:Stage Ⅰ～Ⅱ25.0％,0,41.7％,33.3％(P＞ 0.5)，Stage Ⅲ～Ⅳ 67.7％,57.4％,73.2 ％,32.0％(P＞0.5). Conclusions:Age does not affect the prognosis of patients with ovarian epithelial cancer greatly.General status is more import ant in the patients' response to chemotherapy.Radical surgical treatment and che motherapy can be used on aged with ovarian epithelial cancer if supporting thera py is given.Aged with ovarian epithelial cancer relapse,their prognosis may be very poor.
