胫骨前挫伤难愈性创面不同换药方法的临床观察

目的 观察A组(传统换药组)、B组(综合换药组)、C组(新型敷料组)治疗胫骨前挫伤难愈性创面的临床疗效.方法 选取胫骨挫伤致难愈性创面患者47例,随机分为A组15例(传统换药法)、B组17例(综合换药法)和C组15例(新型敷料换药法),观察不同组别的临床疗效、愈合时间、治疗总费用、换药总次数、创面愈合率的差异.结果 3组总有效率比较,差异无统计学意义(P>0.05);B组和C组痊愈率均优于A组(均P<0.05);C组疗效最好.3组愈合时间、治疗总费用、换药总次数、平均创面愈合率比较,差异有统计学意义(P<0.05);C组在愈合时间、创面愈合率、换药次数优于A、B组;A组治疗总费用最少.结论 相同疗效的基础上,传统换药法总费用较其他两组少,但疗程长、换药次数多;新型敷料换药法疗程短,伤口愈合快,但治疗的总费用较高,推广应用有局限性;综合换药法采用中药配合湿性愈合理念,疗效好、费用适中,更适应发展中医中药的思路和广大患者的需求,具有更好的发展空间,值得临床推广.

The clinical observation on different methods for the treatment of refractory wound surface of tibia

Objective To observe the clinical effect of refractory tibia of Group A(traditional medicine-changing group),Group B(combined therapy group)and Group C(new dressing group).Methods A total of 47 patients were randomly divided into Group A(traditional medicine-changing group)with 15 cases,Group B(combined therapy group) with 17 cases and Group C(new dressing group)with 15 cases.The clinical effect,time of healing,total cost of treatment,total replacement and wound healing rate of of different group was observed.Results There was no statistical significance in terms of the total effective rate of the three groups(P>0.05).The curative rate of Group B and Group C was superior to that of Group A(P<0.05).The curative effect of Group C was the best.There were no statistical significances in terms of the differences of healing time,total cost,total number of changes and average wound healing rate of the three groups according to the retrospective analysis(P<0.05).The healing time,wound healing rate and change of medicine of Group C was the best.The total cost of Group A was the least.Conclusions On the basis of the same curative effect,the total cost of the traditional medicine-changing method was less than that of the other two methods,but with more curative duration and more numbers of medicine-changing.The new dressing group is shown short period and fast wound-healing,with more cost and limited popularization and application.Comprehensive treatment method adopted the concept of Chinese traditional medicine combined with wet healing, with good curative effect and affordable cost,which better meets the development of the thinking of traditional Chinese medicine and the needs of patients,it has better development space and is worth clinical promotion.
