摘    要:目的 去脂体质量指数(fat-free mass index,FFMI)的减少是COPD的重要预估因素,本研究的目的主要是分析COPD患者营养状态与FFMI的关系.方法 根据2016年慢性阻塞性肺疾病全球倡议中COPD分组,选择分在C和D组的84例COPD男性患者,膳食摄入用食物频率问卷,评估身高、体质量、中上臂肌围(MUAC).数据由SPSS 18.0分析.结果 平均年龄(65.7±9.8)岁,所有患者中(21例)25.0%低于正常体质量(BMI<21 kg/m2),(30例)35.7%的患者FFMI低于正常,FFMI< 16 kg/m2;与正常FFMI相比,在低FFMI患者中日常奶制品(t=-3.71,P=0.004)、红色肉类(t=-2.17,P=0.033)、水果(t=-3.97,P=0.002)摄入明显减少,正常FFMI的患者身高(t=-2.35,P=0.021)、体质量(t=-9.16,P=0.000)、MUAC(t=6.68,P=0.000)明显升高.结论 重视低FFMI的COPD患者,并评估营养状况、制订膳食策略,从而改善疾病的预后.

Abstract:Objective Fat-free mass index (FFMI) depletion is an important predictor in chronic obstructive pulmonary disease (COPD) patients.The aim of the study was to analyze the relation of nutritional status of stable COPD patients and FFMI profiles.Methods We investigated 84 male COPD patients in C and D group,based on the 2016 Global Initiative for Chronic Obstructive Lung Disease.Nutritional intake was assessed by food frequency questionnaire.Weight,height,mid-upper arm circumference (MUAC) were taken by a trained dietitian.The data was analyzed with SPSS 18.0 software.Results The mean age of the patients was (65.7 ±-9.8) years.25% of the patients was underweight (BMI<21 kg/m2) and 35.7% had a low FFMI (FFMI<16 kg/m2).Mean daily consumptions of milk-yogurt (t =-3.71,P =0.004),red meat (t =-2.17,P =0.033) and fruits (t =-3.97,P =0.002) were significantly low in the low FFMI group compared to normal FFMI group.Patients with normal FFMI had significantly higher weight (t =-9.16,P =0.000),height (t =-2.35,P =0.021),MUAC (t =6.68,P =0.000) than the patients with low FFMI (all P <0.05).Conclusions We should think highly of COPD patients with low FFMI and evaluate the nutritional status,therefore plan nutritional strategies to improve prognosis of the disease.
