摘    要:目的 探讨动态随访及网络平台健康教育在COPD患者随访中的应用效果.方法 选择2013年6月至2015年6月病情趋稳即将出院的116例COPD患者为研究对象,采用随机数字表法分为观察组和对照组各58例.对照组采用常规出院指导,观察组采用动态随访及网络平台健康教育.采用肺功能、呼吸困难分级(MMRC)及圣·乔治呼吸问卷(SGRQ)评估调查2组干预效果.结果 干预6个月后,观察组FEV1/FVC、PaO2、SaO2均明显高于对照组,PaCO2、呼吸频率(RR)明显低于对照组(t=4.607、4.370、2.720、5.642、3.817、4.468,P<0.05);观察组呼吸困难0级、1级百分率(15.52%、41.38%)明显高于对照组,3级、4级百分率(3.45%、8.62%)明显低于对照组(x2=7.004、6.915,P<0.05);SGRQ量表中疾病症状、活动受限、疾病影响维度各得分及总分均明显低于对照组(t=6.994、3.551、2.966、4.489,P<0.01).结论 动态随访及网络平台健康教育能改善出院后COPD患者肺功能,缓解呼吸困难,提高患者的生活质量.

Abstract:Objective To investigate the application effect of dynamic follow-up visit and health education via network platform in follow-up visit of patients with chronic obstructive pulmonary disease (COPD).Methods One hundred and sixteen cases COPD patients with stabilizing conditions from June 2013 to June 2015 were divided into observation group and control group.Patients in the control group were performed routine discharge guidance,while patients in the observation group were performed dynamic follow-up visit and health education via network platform.The intervention effects of the two groups were assessed and investigated by pulmonary function,modified british medical research council (MMRC)and St George's respiratory questionnaire (SGRQ).Results After 6 months of intervention,FEV1 (forced expiratory volume in 1 second)/FVC(forced vital capacity),arterial partial pressure of oxygen (PaO2) and oxygen saturation(SaO2) in the observation group were significantly higher than in the control group,partial pressure of carbon dioxide in artery (PaCO2) and respiratory rate(RR) were significantly lower than in the control group(t =4.607,4.370,2.720,5.642,3.817,4.468,P <0.05).The percentages of level 0 and level 1 dyspnea of the observation group were 15.52% and 41.38% respectively,which is significantly higher than that of the control group;and the percentages of level 3 and level 4 dyspnea of the observation group were 3.45 % and 8.62 % respectively,which is significantly lower than that of the control group (x2 =7.004,6.915,P <0.05).Scores of disease symptoms,limitation of motion and disease influence dimensions and total score in SGRQ were significantly lower than in the control group (t =6.994,3.551,2.966,4.489,P <0.01).Conclusions Dynamic follow-up visit and health education via network platform can improve pulmonary function of COPD patients after discharge,relieve dyspnea and enhance living quality of patients.
