彩色多普勒超声对脑梗死患者颈动脉病变的研究

目的探讨老年人脑梗死与颈动脉病变之间是否存在相关关系,并了解与脑梗死有关的颈动脉病变指标。方法采用病例对照研究,对51例脑梗死患者(脑梗死组)和25例非脑梗死患者(对照组)进行颈动脉彩色多普勒超声检查,检查双侧颈总动脉、颈内动脉的内膜中层厚度(IMT)、斑块情况、动脉狭窄程度和有关血流参数。结果脑梗死组颈总动脉IMT、颈动脉斑块检出率、斑块积分、软斑及复合斑所占比例高于对照组;脑梗死组颈总动脉和颈内动脉舒张期末流速低于对照组。结论脑梗死与颈动脉病变可能存在相关关系。颈动脉IMT增厚、软斑和复合斑发生率高、舒张期血流速度降低对脑梗死的预测和防治可能有所助益。

Study on carotid lesion in cerebral infarction cases by color Doppler ultrasonography

Objective To explore if there is relationship between carotid lesions and cerebral infarction in elderly and attempt to screen out the indexes of carotid lesions related to cerebral infarction. Methods Color Doppler ultrasonography was applied to 51 cases of cerebral infarction group and 25 cases of non-cerebral infarction group.The intima-media thickness(IMT),the degree of stenosis,carotid plaque and the index of change in the blood flow status of bilateral common carotid arteries and internal carotid arteries were examined.Results The IMT of common carotid arteries,the detection rate of carotid plaque,the plaque scores and the rate of soft and compound plaques in the infarction group were significantly higher than those in the control group. The end diastolic velocities in the common carotid and internal carotid arteries were lower in the infarction group than in the control group. Conclusion It was likely to have relationship between carotid lesions and cerebral infarction. The IMT,the rate of the soft and compound plaques and the end diastolic velocity were valuable indexes of carotid lesions related to cerebral infarction.
