DTI中不同b值对脊髓型颈椎病的诊断价值分析

摘    要:目的研究探讨弥散张量成像(diffusiontensorimaging,DTI)中不同b值对脊髓型颈椎病的诊断价值。方法选取CsM患者和健康者各25例作为CsM组与对照组,对照组采用DTI检查,其中b值分别为0、100、200、400、600、800、1000s/mm2,用质量指数和信噪比评价图像质量。选择图像质量最好的b值对CSM组患者进行扫描并分析图像效果。结果质量指数和信噪比随着b值升高而变小,b=200、400、600s/mm2时质量指数组间差异较小(P〉0.05),而信噪比逐渐变小(P〈O.05);b=800、1000s/mm2时质量指数较小。与对照组相应部位相比,CSM组患者脊髓受压处FA值较低(t=-7.881,P=0.000),平均ADC值较高(t=6.938,P=0.000)。DTI显示的AIX;图和FA图图像清晰无扭曲。结论b=600s/mm2时DTI对脊髓成像效果最好,对CSM的诊断价值较高。

Abstract:Objective To explore the diagnostic value of different b values of dif{usion tensor imaging (DTI) in cer- vical spondylotic myelopathy. Methods A total of 25 patients with cervical spondylotic myelopathy (CSM) and 25 healthy participants were studied. DTI with b values of 0, 100,200,400,600, 800, 1000 s/mm2 were applied in control group. The best value from the previous test was applied for CSM patients scanning. The DTI features in these two groups were compared and analyzed. Results With the increased b value, the quality index and signal to noise ratio (SNR) decreased, but the distinction of quality index among DTI with b = 200,400, 600 s/ram2 had no significant differ- ence (P~0. 05), and SNR decreased (P〈0. 05). DTI with b = 800, 1000 s/rnm2 had lower quality inde~ FA values o{ spinal cord compression were significantly lower and average ADC value were higher in the CSM group than those in the control group (t = -7. 881, P= 0. 000; t = 6. 938, P = 0. 000). ADC pictures and FA pictures were clear in DTI. Conclu- sion Cervical spinal cord DTI with b value of 600 s/mm2 provides best image Gualitv and effectiveness in CSM diagnosis.
