早期高压氧和婴儿抚触辅助治疗新生儿缺血缺氧性脑病的临床观察

目的评价早期高压氧和婴儿抚触综合辅助治疗新生儿缺血缺氧性脑病（HIE）的疗效,为临床治疗提供参考依据.方法将62例中、重度HIE患儿随机分为治疗组和对照组,2组均给常规治疗,实验组早期（72h）高压氧和婴儿抚触综合辅助治疗,观察比较2组治疗后主要临床疗效、症状的缓解时间,治疗后7 d、15d新生儿行为神经测定的评分（NBNA）比较.结果与对照组比较,实验组意识状态、原始反射、肌张力、CT脑水肿消退时间较少（P〈0.01）,7 d NBNA评分（P〈0.05）、15 d NBNA评分（P〈0.01）评分明显优于对照组.结论早期高压氧和婴儿抚触综合辅助治疗对新生儿缺氧缺血性脑病有显著的临床治疗作用,值得临床推广.

Clinical Observation of Early Hyperbaric Oxygen Therapy and Infant Massage in Adjunctive Treatment of Neonatal Hypoxic Ischemic Encephalopathy

Objective To evaluate the effect of early hyperbaric oxygen therapy and infant massage in adjunctive treatment of neonatal hypoxic ischemic encephalopathy.Methods Sixty-two children patients with moderate to severe HIE were randomly divided into treatment group and control group. Patients in both groups were given conventional treatment,and additional early experimental group（72h） and infant massage were given as adjunctive treatment to patients in the treatment group. The clinical effects, symptoms relieving time,7d and 15 d neonatal behavioral neurological assessment（NBNA） scores were compared between two groups. Results Compared with the control group,in the experimental group,the consciousness, primitive reflexes,muscle tone,and CT cerebral edema regression time were decreased（P〈0.01）,7d NBNA score（P〈0.05） and 15 d NBNA score（P〈0.01） were significantly higher.Conclusion Early hyperbaric oxygen therapy and infant massage have significant clinical effects in adjunctive treatment of neonatal hypoxic ischemic encephalopathy,so it is worthy of promotion.
