摘    要:探讨快速、准确检测临床常用标本真菌分离、鉴定方法。方法PCR与RFLP相结合,应用9种临床重要真菌的通用引物,对常用临床标本(痰、BALF、胸水、尿、血)224份进行分离、鉴定,并对19例进行痰与非痰标本结果一致性、敏感性比较。 结果224份标本共检出真菌83株,阳性率37.03%,其中白色念珠菌35株(42.3%),假热带念珠菌30株(36.1%),热带念珠菌10株(12%),光滑念珠菌8株(9.6%)。19例痰与非痰标本47次检测,一致性、敏感性良好。结论PCR结合RFLP对真菌检测具有敏感、快速的特点,值得临床推广应用。

Abstract:Objective:To develop a rapid and accurate method for isolation and identification of fungi in samples detecting medically important fungi for clinical use.Methods:Clinical samples such as sputum, BALF, blood,urine and pleurorrhea were tested by a combination of polymerase chain reaction (PCR) and restriction fragments length polymorphism (RFLP)and with universal primers for 9 inportant clinical fungal species. PCR RFLP was also used in detecting fungi in different kinds of samples, sputum and non sputum, and their sensitivity and correlation were compared. Results:83 strains were detected from 224 samples, the positive rate was 37.03%. Among them were 35 strains (42.3%) of Candida albicans, 30 strains of C.pseudotropicalis( 36.1% ), 10 strains of C. tropicalis(12%)and 8 strains of T. glabrata(9.6%). The sensitivity and correlation were satisfiying in 47 sputum and non sputum samples from 19 patients. Conclusions:PCR RFLP is a rapid and sensitive method in detecting medical fungi, and shows a good prospect in clinical use.
