血管内皮生长因子和P53及细胞增殖核抗原蛋白表达与垂体腺瘤复发的关系

摘    要:目的 探讨血管内皮生长因子(VEGF)、P53和细胞增殖核抗原(PCNA)蛋白表达与垂体腺瘤的侵袭性和复发的关系。方法 应用免疫组化染色方法检测VEGF、P53和PCNA蛋白在30例侵袭性垂体腺瘤及30例非侵袭性垂体腺瘤中的表达。结果 侵袭组中VEGF和PCNA蛋白的表达阳性率分别为16/30(53.3%)和13/30(43.3%),非侵袭组中分别为8/30(26.7%)和5/30(16.7%),两种蛋白的表达水平侵袭组明显高于非侵袭组(P<0.05)。P53蛋白表达阳性率在两组中分别为7/30(23.3%)和0/30(0%),两组差异有显著意义(P<0.01)。复发组的VEGF、P53和PCNA蛋白表达阳性率在第一次手术组分别为5/9(55.5%)、2/9(22.2%)、5/9(55.5%),在第二次手术组分别为5/9(55.5%)、5/9(55.5%)和4/9(44.4%);非复发组分别为6/21(28.5%)、0/21(0%)和4/21(19.0%)。复发组VEGF和PCNA蛋白表达与非复发组比较,差异有显著意义(P<0.05),而两次手术组间比较无差异;P53蛋白表达在第一次手术组与非复发组间的差异也无显著意义(P>0.05),但第二次手术组则明显高于非复发组(P<0.01)。结论 VEGF、P53和PCNA蛋白异常表达与垂体腺瘤的发生、侵袭和复发改变有关,检测这3项指标可对肿瘤侵袭和预后进行评价。

Abstract:Objective TO study the relationship between the protein exprssion of VEGF, P53, PCNA and the invasiveness, recurrence of pituitary adenoma. Methods The protein expressions of VEGF, P53, PCNA were detected by immunohistochemical assay in 30 cases of invasive pituitary adenomas and 30 cases of noninvasive pituitary adenomas. Results Positive expression rate of VEGF and PCNA were 16/30 (53. 3% ) andl3/30 (43. 3% ) respectively in invasive group. Those were 8/30 (26. 7% ) and 5/30 (16. 7% ) respectively in noninvasive group. The postive protein expressions of VEGF and PCNA in invasive group were significantly higher than that in noninvasive group (P < 0. 05). The positives staining of P53 protein was found in 23. 3% of invasive pituitary adenomas and 0% of noninvasive adenomas. Positive staining of P53 in invasive group was significantly higher than that in noninvasive group ( P < 0. 01). The positive expression rate of VEGF, P53 and PCNA in the first operation group were 5/9 (55. 5% ) , 2/9 (22. 2% ) ,5/9(55.5% ) respectively, 5/9(55. 5% ) ,5/9(55. 5% ) ,4/9(44.4% ) in the second operation respectively, and 6/21 ( 28. 5% ) , 0/21 (0% ) , 4/21 ( 19. 0% ) in nonrecurrent group respectively. The recurrent group had a significantly higher postive expression of VEGF, PCNA protein than that of the nonrecurrent group (P <0. 05) . There was no significantly difference between the first and second operation group ( P > 0. 05). There was no significantly difference of the positive rate of P53 between the first operation group and the nonrecurrent group (P >0. 05). However, the positive rate of P53 in the second operation group was significantly higher than that of the recurrent group (P < 0. 01 ). Concisions The abnormal expressions of VEGF, P53 and PCNA protein may play important roles in the tumor genesis and progression of pituitary adenomas and can be considered as an indicators of the invasiveness and recurrence in pituitary adenomas.
