嗜铬细胞瘤诊治分析(附39例报告)

目的探讨嗜铬细胞瘤的诊治体会。方法回顾性分析39例嗜铬细胞瘤的临床资料。结果本组均采用开放手术治疗，32例经腰切口，7例经腹切口（其中膀胱嗜铬细胞瘤2例），术中6例先后出现室性心律失常，其中1例出现室颤，术中无死亡病例。术后病检良性35例，恶性4例。结论B超、CT结合临床症状仍是诊断嗜铬细胞瘤主要依据。嗜铬细胞瘤围手术期处理较为复杂，但只要做到术前充分准备，术中与麻醉医师密切配合，术后根据中心静脉压补液扩容，持续动态监测生命体征，患者均能安全渡过围手术期。

Diagnosis and treatment of pheochromocytoma (report of 39 cases)

Objective] To investigate diagnosis and treatment of pheochromocytoma. Methods] Thirty-nine cases of pheochromocytoma diagnosed and treated from 1985 to 2004 were studied retrospectively. Results] All cases received opening operation. Thirty-two cases were cut by lumber incision, seven cases by abdominal incision, (two bladder pheochromocytoma among them). Six cases underwent chamber arrhythmia, of them, only one chamber tremble case, and no cases died in operation. Thirty-five cases were benign, four cases malignant. Conclusions] Diagnosis of pheochromocytoma mainly relied on B ultrasonic, CT and clinical symptoms. Management of pheochromocytoma was complex in perioperation, sufficient preoperative arrangement, cooperation with anaesthetis, transfusion base on central venous pressure, continual life sign inspection, made patients safely get through perioperation.
