院前紧急气管插管配合便携式呼吸器应用的临床观察

目的分析院前紧急气管插管配合简易呼吸机的临床效果,进一步提高患者抢救的成功率。方法选择2008年11月至2011年10月我科院前急救过程中采用气管插管并便携式呼吸器抢救的患者21例为观察组,随机选取同期经气囊辅助呼吸的34例患者为对照组,比较两组患者气管导管置入时间、血氧饱和度、成功率等临床指标。结果两组患者导管置入时间比较差异无统计学意义(P>0.05);观察组血氧饱和度及转运成功率明显优于对照组,差异有统计学意义(P<0.05)。结论急诊科医师需要熟练掌握气管插管的技术,了解转运途中可能出现的情况并能够及时正确地处理,并能够调节合适的模式及参数使用呼吸机维持患者的呼吸功能,提高抢救的成功率。

Clinical Observation on Prehospital Emergency Tracheal Intubation with the Application of Portable Respirator

Objective To investigate the clinical effect of prehospital emergency tracheal intubation with simple ventilator clinical results,to further improve the success rate of rescue.Methods 21 patients were selected from November 2008 to October 2011 of Chang′an Hospital of Dongguan,on which during the course of prehospital care emergency portable respirator with tracheal intubation were applied,34 patients with balloon assisted respiration were randomly selected as control group,clinical indicators such as the tracheal catheter placement time,oxygen saturation,and success rate of the two groups were compared.Results The comparison of tracheal catheter placement time between the two groups showed no statistical difference(P>0.05).Oxygen saturation and transfer success rate of the observation group were better than the control group with statistically significant difference(P<0.05).Conclusion Emergency department physicians need to be skilled in tracheal intubation technique,understand the possible situations during transition,be able to deliver timely and correct treatment,and can adjust the appropriate mode and parameters of a ventilator to maintain the patient′s respiratory function,to improve the success rate of rescue.
