生长激素释放肽在胃黏膜癌变过程中表达变化的研究

目的探讨生长激素释放肽（Ghrelin）在胃癌及其癌前疾病诊断中的价值。方法选择浅表性胃炎患者31例，萎缩性胃炎患者21例，上皮异型增生患者6例，胃癌患者19例，健康志愿者21例，分别进行胃镜检查及胃黏膜活检，应用免疫组化的方法，检测胃黏膜Ghrelin的表达水平。结果①胃黏膜Ghrelin的表达：浅表性胃炎、萎缩性胃炎、上皮异型增生患者Ghrelin积分光密度均低于对照组（P〈0．05），萎缩性胃炎患者、上皮异型增生患者低于浅表性胃炎患者（P〈0．05），伴有上皮异型增生患者与萎缩性胃炎患者比较无显著差异，胃癌患者Ghrelin积分光密度则高于对照组及其他各组（P〈0．05）；②Hp感染：Hp感染阳性DGR患者胃黏膜Ghrelin积分光密度低于Hp阴性组（P〈0．05）；③Hp感染阳性的胃癌组织Ghrelin积分光密度低于Hp感染阴性的胃癌患者（P〈0．05）。结论胃癌患者生长激素释放肽水平的变化对疾病的预后观察具有重要的临床价值。

The study on changes of Ghrelin and its fluencing factors in carcinogenesis of gastric mucosa

Objective To explore the value of Ghrelin in the diagnesis of gastric precancerous lesions and gastric cancers. Methods Gastroscopy, histologic examination of gastric mucosal biopsy sample were performed in 31 patients with chronic superficial gastritis （CSG） ,21 patients with chronic atrophic gastritis （CAG）, 6 patients with gastric epithelial dysplasia （Dys） ,19 patients with gastric cancer and 21 controls. Immunohistochemical staining was used to detect the expression of Ghrelin in gastric mucosa. Results 1. The expression of Ghrelin in CSG. CAG, Dys were lower than that in the control subjects （P 〈 0.05）. The expression of Ghrelin in CSG was higher than that in CAG and Dys The expression of Ghrelin in cancer was higher than that in CSG., CAG, Dys and control, lower than that in IM（P 〈 0.05 ）. 2. The expression of Ghrelin in gastric mucosa in Hp positive group were significantly lower than that in Hp negative group （P 〈 0.05）. 3. The expression of Ghrelin in gastric cancer in tip positive group were significantly lower than that in Hp negative group （P 〈 0.05）. Conclusions The expression of Ghrelin in gastric mucosa was significantly increased in patients with gastric cancer.
