开心胶囊预处理抗犬心肺复苏后心功能不全的实验研究

目的:观察开心胶囊预处理抗犬心跳骤停复苏后心功能不全的作用。方法:选用健康杂种犬15只,随机分为假手术组、模型组和开心胶囊组,每组5只;复制心肺复苏模型。选择开胸前、除颤后即刻、自主循环恢复30min、60min、120min、180min 6个时间点记录各心功能指标;心肌组织切片在光镜和电镜下观察心肌损伤的结构变化。结果:与模型组相比,开心胶囊组HR、MABP下降趋势较缓,保持较高的HR、MABP(P<0.05);开心胶囊组的MPAP、CO在复苏后亦呈下降趋势,PCWP呈持续增高趋势,与模型组相比其MPAP、PCWP仍然维持较低水平,同时能够维持较高的CO(P<0.05)。光镜下模型组心肌染色阳性点总面积增大,与假手术组比较有显著性差异(P<0.05);开心胶囊组阳性总面积降低,与模型组比较有显著性差异(P<0.05);电镜下开心胶囊组虽然出现细胞结构损伤,但损伤程度较模型组轻。结论:开心胶囊对犬心跳骤停复苏后心功能不全具有良好的防治作用。

Study on Kaixin Capsule in Preventing Canine Post-resuscitation Myocardial insufficiency

Objective: To investigate the effect of Kaixin Capsule(KC) preconditioning in preventing canine post-resuscitation myocardial dysfunction.Methods: A total of 15 hybrid canines were randomly divided into pseudo-operation group(group A),model group(group B),and Kaixin capsule group(group C).There were 5 canines in each group.The model of cardiopulmonary resuscitation was established.Indices of cardiac function were examined before opening pectoral cavity,at the very time of post-defibrillation,on the 30th, 60th,120th and 180th minute of recovering circulation.Histological features of injured myocardium were examined under light microscope and electron microscope.Results: Compared with group B,heart rate(HR) and mean arterial blood pressure(MABP) in group C decreased more slowly.Mean pulmonary artery pressure(MPAP) and CO in group C also decreased after resuscitation.Pulmonary capillary wedge pressure(PCWP) in group C continued to increase after resuscitation.MPAP and PCWP in group C were lower than those in group B(P<0.05),while CO in group C kept higher than those in group B.Positive-staining myocardial area in group C decreased,and cellular impairment under electron microscopic was milder in group C as compared with group B(P<0.05).Conclusion: Kaixin Capsule preconditioning has a good effect in preventing and treating post-resuscitation myocardial dysfunction.
