营养不良与COPD呼吸衰竭的临床关系研究

目的探讨营养不良与COPD呼吸衰竭患者的预后及医疗费用等方面的相互关系。方法收集2005年1月～2006年12月因COPD呼吸衰竭入住本院呼吸科的177名患者的临床资料。按血清白蛋白水平（35G／L）分为营养不良组及对照组，分别比较两组的死亡率、医疗费用、呼吸衰竭类型、住院天数等指标。结果177例患者中营养不良66例（血清白蛋白耋35G／L），营养状态正常111例（血清白蛋白〉35G／）。营养不良组死亡率、医疗费用、I型呼吸衰竭比例均高于对照组，差异有统计学意义。结论营养不良与COPD呼吸衰竭有较密切的临床关系，营养不良可增高COPD呼吸衰竭的临床死亡率及医疗费用，COPD患者缓解期营养不良状态的纠正可能有助于改善COPD呼吸衰竭的临床预后及降低医疗费用。

Interrelationship between malnutrition and COPD with respiratory failure

Objective To probe the interrelationship of medical treatment expenses and clinical prognosis between malnutrition and COPD respiratory failure. Methods We collected the clinical record of 177 respiratory failure patients from January 2005 to December 2006. We divided them into two groups： malnutrition group and compared group, by the level serum albumin （35 G/L）. Mortality, treatment expenses, the type of respiratory failure, hospitalized days and others were compared. Results There were 66 patients who had malnutrition （ serum albumin 〈 or = 35 G/L） while another 111 patient were normal （ serum albumin 〉 35 G/L）. The proportion of mortality, treatment expenses and I respiratory failure of the malnutrition group were all higher than that in the comparison group. The result was significant. Conclusion There is a close relationship between malnutrition and COPD respiratory failure. Malnutrition could increase the mortality rate and expenses. Rectification of the condition of malnutrition in the alleviafive period might improve the clinical prognosis and decrease the treatment fees.
