下尿路梗阻对膀胱肿瘤术后复发的影响

目的：探讨下尿路梗阻对膀胱移行细胞癌（BTCC）术后复发的影响。方法：回顾性分析82例BTCC并发下尿路梗阻患者的临床资料，其中48例行单纯膀胱部分切除术（开放组），34例行经尿道膀胱肿瘤电切术（TURBT）加前列腺电切术（TURP）或膀胱颈部电切开术（BNI）（腔内组）。结果：随访2-6年，开放组及腔内组复发率分别为39.6％和17.6％，两组比较差异有统计学意义（P〈0.01）；两组平均复发时间比较差异无统计学意义（P〉0.05）。结论：下尿路梗阻是BTCC复发的重要因素，对BTCC并发下尿路梗阻者，解除梗阻可降低BTCC的复发率。

Analysis of Lower Urinary Tract Obstruction on Recurrence of Bladder Cancer after Surgical Ablation

Objective：To study the effects of lower urinary tract obstruction on recurrence of bladder cancer after surgical ablation. Methods：The clinical data of 82 cases of patients with bladder transitional cell carcinoma and lower urinary tract were retrospectively reviewed. Of the 82 patients, 48 underwent partial cysteetomy （Open group）, 34 had TURBT and TURP or BNI（Endourological group）. Results：The follow-up period ranged from 2-6 years. Recurrence rate of Open and Endourological groups is 39.6% and 17.6% respectively, significant differences were observed in two groups （P〈0.01）, and there were no significant differences in mean interval of recur-rence （P〉0.05）. Conclusions：lower urinary tract obstruction is an important factor to the recurrence of bladder transitional cell carcinoma. To the patients of bladder cancer with lower urinary tract obstruction, relief of lower urinary tract obstruction would decrese recurrence rate of bladder cancer.
