| 摘 要： | 上海特色的社区卫生服务模式，是根据社会人群卫生状况及人口年龄结构，构建由社区卫生服务中心、社区卫生服务站、社区老年服务（护理中心）组成的基层卫生服务体系。而作为医疗卫生服务网底的社区卫生服务站，以健康档案为基础，以预防保健为先导，因地制宜地开展医疗护理工作，逐步地实现其“六位一体”的功能定位。
