血浆EBV DNA检测对鼻咽癌的诊断作用

目的探讨血浆EBV DNA检测对鼻咽癌的诊断作用。方法将2006年12月至2007年3月在我科就诊、病理确诊的65例鼻咽癌作为治疗前组，经我院放疗科治疗后，分别于放疗结束后3、5、8个月来我科系统复查的病人作为治疗后组，将同期行健康体检的29例作为对照组，共3组。采用荧光定量PCR方法对3组的空腹血浆EBV DNA进行检测。结果3组间血浆EBV DNA阳性率比较有统计学意义（P〈0．05）；治疗后组的阳性病例全部有鼻咽癌复发，阴性病例没有复发；有、无淋巴结转移组间阳性率比较差异有统计学意义（P=0．00）。结论血浆EBV DNA是鼻咽癌诊断的重要分子标记，其阳性的病例有较高淋巴结侵袭发生率。

Diagnosis of NPC with Plasma EBV DNA

Objective To evaluate the value of using plasma Epstein-Barr virus （EBV） DNA in the diagnosis of nasopharyngeal carcinoma （NPC）. Method 65 NPC patients were enrolled from December, 2006 to March, 2007 for this study. The patients were treated firstly with radiotherapy and then further received the treatment at 3, 5, and 8 months after radiotherapy. 29 normal subjects were enrolled as control. Concentration of plasma EBV DNA was detected by real-time quantitative polymerase chain reaction （real-time PCR）. Result The levels of plasma EBV DNA were significantly different between the treatment and the control groups （P〈0.05）. The presence of plasma EBV DNA and involvement of lymphnodes metatasis were correlated with the disease recurrence. Conclusion Plasma EBV DNA may be used in the diagnosis of NPC, and the positive detection of plasma EBV DNA is an indicator of metatasis to lymphoid organs.
