成年人肠套叠诊治体会

目的总结成人肠套叠的病因、临床特征和诊治方法，提高对成年人肠套叠的认识.方法对2010年1月至2013年1月郴州市第一人民医院收治的成年人肠套叠的临床资料进行回顾性分析。结果35例患者中，临床表现为腹痛者35例．恶心呕吐者16例，肛门排气排便停止或减少者10例，黑便或血使者8例，腹部包块者3例；所有患者均行CT检查，术前确诊23例；31例行手术的患者中28例为肠道肿瘤，其中良性肿瘤占20例，恶性肿瘤占8例。结论病理因素是成年人肠套叠的主要戍因；CT检查的特异性征象对成年人肠套叠的诊断敏感性较高；成年人肠套叠首选手术治疗：腹腔镜可作为成年人肠套叠的首选诊断及治疗方法。

Diagnosis and treatment of intussusception in adults

Objective To summanze the causes,the clinical characteristics and the treatment methods of intussuscep- tion in adults,and to improve the cognition of intussusception in adults. Methods Retrospective analysis was conducted on clin- ical data of adult intussusception treated in Chenzhou municipal first people＇s hospital from January 2010 to January 2013. Re- suits In all the 35 patients,35 eases had abdominal pain; 16 cases were with pernicious vomiting;flatus and defecation were stopped or dec, teased in 10 eases;8 cases had melena or bloody stool;3 cases had abdominal mass. CT examination was per- formed on all the patients ,and 23 cases were diagnosed with intussusception before operation. In the 31 cases underwent opera- tion,28 eases were the intestinal tumor including 20 eases of benign tumor and 8 eases of malignant tumor. Conclusion Patho- logical ｜actors are the main causes of adult intussusception. The specific sign of CT examination has superior sensitivity in diag- nosing of intussusception in adults. Operative treatment is the first choice for intussusception in adults. Laparoscopy can be used am the first choice of diagnosis and therapy of adult intussuseeptions.
