乌司他丁治疗老年重症急性胰腺炎的临床观察

目的观察乌司他丁(Ulinastatin,UTI)对早期老年重症急性胰腺炎(severe acute pancrea-titis,SAP)的临床疗效。方法69例老年SAP患者随机分成两组:UTI组38例,应用乌司他丁治疗;对照组31例,采用奥曲肽治疗。结果组内比较,白细胞计数第10d较第1d明显降低(P<0.05),血清淀粉酶及CT所示胰腺坏死范围第5d、第10d的结果较第1d明显降低和缩小(P<0.05),且与患者的临床表现相一致。但组间比较无统计学差异(P>0.05)。UTI组治疗有效率为86.8%(33/38),并发症发生率为63.2%(24/38);奥曲肽组治疗有效率为90.3%(28/31),并发症发生率为54.8%(17/31)。两组比较均无统计学差异(P>0.05)。结论乌司他丁治疗早期老年SAP有肯定的疗效。

Application of Ulinastatin in Treatment of Severe Acute Pancreatitis in Aged Patients

Objective To investigate the therapeutic effect of Ulinastatin(UTI) in treatment of severe acute pancreatitis(SAP) in aged patients. Methods 69 aged cases of SAP were randomly divided into 2 groups, 38 cases received UTI treatment assigned to group A, rest 31 cases assigned to group B received Octreotide treatment instead of UTI. Results Comparation in each group, in terms of WBC count was obviously lower at day 10 than at day 1 (P<0.05), in terms of serum amylase and pancreatic necrotic range showed decreasing at day 5 and day 10 than at day 1 (P<0.05), in consistent with the clinical presentation. The difference between two groups was not significant (P>0.05). The effecive rate was 86.8% vs. 90.3% and complication rate were 63.2% vs. 54.8% in group A and B respectively, showing no significant difference(P>0.05). Conclusion UTI demonstrates a confirmed therapeutic effect on SAP in aged patients.
