影响经腹输卵管结扎术高危因素临床分析

目的:探讨高危因素对经腹输卵管结扎术的影响,提高手术成功率及安全性。方法:对202例行输卵管结扎术病例进行回顾性对照分析,分为无高危因素组90例,高危因素组112例,高危因素包括:有下腹部手术史42例、多次流产史(≥3次)28例、哺乳期39例、高龄(≥40岁)24例。结果:在高危因素组中有下腹部手术史者、多次流产史、高龄术中发生粘连情况明显多于无高危因素(P<0.05),手术时间明显长于无高危组(P<0.05),有5例因粘连严重致一侧结扎失败。哺乳期无明显影响。结论:有下腹部手术史、多次人流史、高龄对行输卵管结扎术成败有较大影响。

Clinical analysis on risk factors of transabdominal tubal ligation

Objective:To explore the effect of high risk factors on transabdominal tubal ligation,increase the success rate and safety.Methods:202 cases who underwent transabdominal tubal ligation were analyzed retrospectively.They were divided into non-high risk factor group(90 cases) and high risk factor group(112 cases),the high risk factors included the history of lower abdominal surgery(42 cases),the history of multiple abortions(≥3 times,28 cases),lactation period(39 cases) and advanced age(≥40 years old,24 cases).Results:The incidence of intraoperative adhesion in high risk factor group with the history of lower abdominal surgery,the history of multiple abortions and advanced age was higher than that in non-high risk factor group(P<0.05);the operative time in high risk factor group was significantly longer than that in non-high risk factor group(P<0.05),unilateral ligation failure occurred in 5 cases because of severe adhesion.Lactation period was not an effect factor of transabdominal tubal ligation.Conclusion:The history of lower abdominal surgery,the history of multiple abortions and advanced age have a great impact on the success of transabdominal tubal ligation.
