联合检测WT1基因和COX-2基因表达在急性白血病的临床价值

摘    要:目的 通过观察急性白血病(AL)患者WT1基因(WT1 mRNA)和COX-2基因(COX 2 mRNA)的表达情况,探讨其相关关系及临床检测价值.方法 125例初诊AL患者为研究对象,采用实时荧光定量PCR(RQ PCR)动态检测WT1mRNA和COX-2 mRNA的表达水平,并分析其与临床参数的相关性.结果 初诊AL患者WT1 mRNA和COX-2 mRNA表达水平(中位数157.7和163.2)均显著高于正常对照组(中位数1.65和0.87) (P<0.01),且WT1 mRNA与COX-2mRNA表达水平呈相关性(r=0.509,P<0.05).初诊AL患者WT1 mRNA和COX-2 mRNA的表达水平显著高于缓解组(中位数3.65和4.37)(P<0.05);缓解组AL患者WT1 mRNA和COX-2 mRNA的表达水平与对照组的差异无统计学意义(P>0.05);复发组AL患者WT1 mRNA和COX-2 mRNA的表达水平(中位数167.9和178.5)与初诊组的表达水平差异无统计学意义(P>0.05).WT1 mRNA表达水平在AL各亚型之间差异有统计学意义(X2=423.567,P<0.01),M5WT1 mRNA表达水平最低,M3表达最高.结论 AL患者WT1基因与COX-2基因表达水平存在相关性,联合检测WT1 mRNA和COX-2 mRNA,可作为AL患者疗效评价、监测微小残留病和预后判断的指标,为临床化疗方案个体化提供更可靠的依据.

Combined Detection of WT1 Gene and Cox-2 Gene Expression in the Clinical Value of Acute Leukemia

Abstract:Objective The present study is aimed to investigate the correlation between WT1 gene (WT1 mRNA) and COX-2 gene (COX-2 mRNA) in patients with acute leukemia (AL) and their values in clinical detection by observing their expressions.Methods 125 cases of newly diagnosed AL patients were selected as the objects of study.The expression levelsof WT1 mRNA and COX-2 mRNA were dynamically detected by using real time fluorescent quantitative PCR (RQ-PCR).Their correlations with clinical parameters were also analyzed.Results The expression levels of WT1 mRNA and COX-2 mRNA in the newly diagnosed group (median,157.7 and 163.2) were significantly higher than the control group (median,1.65 and 0.87) (P<0.05).And there showed a correlation between the expression levels of WT1 mRNA and COX-2 mR-NA (r=0.509,P<0.05).The expression levels of WT1 mRNA and COX2 mRNA in the newly diagnosed group were significantly higher than the remission group (median,3.65 and 4.37)(P<0.05).There were no significant differences in the expression levels of WT1 mRNA and COX 2 mRNA between the remission group and the control group (P>0.05).There were no significant differences in the expression levels of WT1 mRNA and COX 2 mRNA between the relapse group (median,167.9 and 178.5) and the newly diagnosed group (P>0.05).The expression levels of WT1 mRNA were significantly different among various subtypes of AL (x2 =423.567,P<0.01).The expression levels were the lowest in M5 and the highest in M3.Conclusion There was a correlation between the expression levels of WT1 gene and COX 2 gene in AL patients.The joint detection of WT1 mRNA and COX2 mRNA could be used as an indicator of efficacy evaluation,monitoring of minimal residual disease and prognosis in AL patients,and provide more reliable basis for the individualization of clinical chemotherapy regimens.
